2型糖尿病合并脑梗死的危险因素分析

摘　　要：目的探讨2型糖尿病合并脑梗死患者的颈动脉中层厚度(IMT)与危险因素。方法测定106例2型糖尿病合并脑梗死(DM+CI)及101例单纯糖尿病(DM)患者的颈动脉IMT,对两组间临床数据进行比较及回归分析。结果DM+CI组的年龄、高血压病比率、尿微量白蛋白排泄率比单纯DM组明显升高,DM+CI组的平均IMT比单纯DM组明显增厚,随着颈动脉IMT的增厚CI发生危险性增大。回归分析显示,年龄、高血压、高甘油三酯是DM+CI的独立危险因素。结论2型糖尿病颈动脉IMT与脑梗死有明显关系,2型糖尿病患者,年龄、高血压、高甘油三酯是发生脑梗死的独立危险因素。Objective To study the carotid artery intima-media thickness （IMT） in type 2 diabetic mellitus with cerebral infarction and the relationship of it with other risk factors.Methods Carotid IMT was measured in 106 patients with type 2 diabetic mellitus associated with cerebral infarction,101 patients without cerebral infarction; the clinical data were compared and logistic regression analysis was performed to reveal the risk factors for cerebral infarction. ResuIts The old age, hypertension, duration of diabetes and abnormality of urinary albumin excretion rate （UARE） were more common in patients with cerebral infarction as compared with those without cerebral infaction. Mean carotid 1MT in type 2 diabetic mellitus with cerebral infarction were higher than that in type 2 diabetic mellitus without cerebral infarction, the risk of cerebral infarction increased gradually with increasing of carotid IMT. With stepwise multivariate logistic regression analysis, old age,hypertension and hypertriglyceridemia were identified as significant and risk factors for type 2 diabetic mellitus associated with cerebral infarction. Conclusion Increased carotid IMT is associated with cerebral infarction in type 2 diabetic mellitus; in type 2 diabetic mellitus, old age, hypertension and hypertriglycerldemia are the risk factors for cerebral infarction.
