腹泻型肠易激综合征患者心理健康状况分析

摘　　要：目的探讨陆军官兵腹泻型肠易激综合征(Irritable bowel syndrom,IBS)患者的心理健康状况、心理社会因素、应对方式及相关因素。方法对72例腹泻型IBS患者进行症状自评量表(SCL-90)、抑郁自评量表(SDS)、焦虑自评量表(SAS)、应对方式问卷和一般情况调查分析。结果72例腹泻型IBS患者的SCL-90总均分与中国常模均分比较有显著性差异(P<0.01),总均分≥2.0分占84.72%,总均分≥3分占34.72%。焦虑自评量表20项总分均值与中国正常人常模相比有显著性差异(P<0.01)。应对方式以消极应对为主。多因素分析结果表明,与腹泻型IBS明显相关的因素是部职别、军龄、一年内是否有心理应激事件、自感精神紧张等。结论陆军官兵腹泻型IBS患者的心理健康状况令人担忧,其发生发展与社会心理因素、应对方式密切相关,因此陆军官兵腹泻型IBS的治疗要兼顾患者的生物和心理社会两个方面,加强心理治疗和精神药物治疗。Objective To investigate the status of mental health and the social mental factor, copying styles and the associated factors in army officers and soldie：s patients with diarrhea-predominant irritable bowel syndrome （D-IBS）.Methods 72 D-IBS patients was tested with Symptom Checklist 90 （SCL-90）, Self-rating depression scale（SDS）, Self-rating anxiety scale （SAS）, copying styles questionnaire and general condition. Result The average scores of every factors of SCL-90 were significant higher than those of China Norm（P〈0.01）, the average score≥2.0 had 84.72%, ≥3.0 had 34.72%.The average scores of SAS were significant higher than those of China Norm （P〈0.01）.lt was predominantly passive how to copy with diseases. The result of multiple factors analysis showed the factors that was associated with D-IBS was official rank （soldier）, time in army （≤years）, the events of psychological stress and self-feeling nervous in 1 year or not.Conclusion It orderes anxious that status of mental health of army officers and soldiers D-IBS patients, which is one of the important factors that the members of troops are decreased in non-fight, its occurrence and development is associated with the social mental factors, ＂copying styles. It is the important for therapy of army officers and soldiers patients with D-IBS giving attention to the both sides of biology and psycho-soclety and strengthening ＂psychotherapy and psychotropic drug treatment.
