支气管灌洗联合机械通气对难治性哮喘的疗效观察

摘　　要：目的探讨纤支镜支气管灌洗术(FOBwL)联合机械通气在难治性哮喘治疗中的作用。方法对41例难治性哮喘患者在常规治疗后,给予呼气末正压通气及纤支镜支气管灌洗治疗后,观察治疗前后症状、体征、通气功能(FVC、FEV1、PEF)及血气。结果患者上述指标明显改善,治愈39例,2例死亡;治疗前后通气功能及血气有显著性差异(P<0.05)。结论支气管内炎症细胞浸润及黏液痰栓堵塞是哮喘难以缓解的重要因素之一,机械通气联合支气管灌洗治疗可显著改善难治性哮喘患者的肺通气功能,有效缓解哮喘症状。Objective To study the effect of combining fibroptic bronchoscopy with lavage （FOBwL） with mechanical ventilation in patient with the refractory asthma. Methods 41 patients with refractory asthma who had no obvious remission after routine treatment were established to enforce mechanical ventilation and FOBwL were performed to clear respiratory sputum and inject drugs. Results. we can found symptom, sign, the blood gas and spirometric measurements of FVC, FEV1 and PEF increased significantly （P 〈 0.05）. Conclusion Inflammation of endobronchial and mucous impaction may be cause in asthmatic exacerbation. Combined FOBwL with mechanical ventilation can clear inspissated secretions and improve aeration of lung with apparent efficacy. It can relieve dyspnea of patients.
