载脂蛋白E基因多态性对高脂血症痰瘀互阻证患者血脂浓度及应证方药调脂疗效的影响

摘　　要：目的探讨中国北方汉族人群载脂蛋白E基因多态性对高脂血症痰瘀互阻证血脂浓度及应证方药(调脂通脉颗粒)调脂疗效的影响。方法172例高脂血症痰瘀互阻证患者随机分为治疗组(调脂通脉颗粒)和对照组(血脂康胶囊),观察症状、体征(证候积分)和血脂浓度的变化,应用PCR-RFLP技术进行ApoE基因型测定。结果调脂通脉颗粒调脂疗效与血脂康相当,两组差异无显著性意义(P>0.05),但在证候积分改善上治疗组显著优于对照组(P<0.01);携带ApoEE3/3和E3/4基因型高脂血症痰瘀互阻证患者的低密度脂蛋白胆固醇(LDL-C)浓度明显高于E2/3基因型者(P<0.05或P<0.01);E3/4型患者对应证方药在LDL-C浓度的改善上较E2/3和E3/3型明显(P<0.01)。结论高脂血症痰瘀互阻证患者ApoE基因第4外显子不同基因型个体对应证方药调脂作用的敏感性不同,表明遗传因素在其中发挥着重要作用。Objective To explore the effects of apolipoprotein E gene polymorphism on serum concentration of lipids and lipid-adjusting effect of correspond medicine in hyperlipidemia patients with phlegm and blood stasis syndrome in the Han nationality in north china. Methods Totally 172 cases of hyperlipidemia patients with phlegm and blood stasis syndrome were randomly divided into observation group （n = 87） and control group（n= 85）. The observation group was treated by Tiaozhi Tongmai Granule. The control group was treated by Xuezhikang Capsule. Clinical symptom and serum concentration of lipid were observed before and after treatment. Using PCR-RFLP methods, we analyzed the forth extron genotypes of ApoE gene in 172 cases. Results In hyperlipidemia patients with phlegm and blood stasis syndrome, serum concentration of lipid in observation group and control group were greatly improved after treatment. The total effective rate of observation group was superior to that of control group （P〈0.01）. Before treatment, serum LDL-C concentration of ApoE E3/3 and E3/4 genetype was significantly higher than that of the E2/3 （P〈0. 05- 0. 01）. The serum LDL-C concentration of E3/4 genetype was more significantly improved than that of the E2/3 and E3/3 genetype （P〈0. 01）. Conclusion Sensitivity of lipid- adjusting effect of correspond medicinediffered due to different ApoE genotype in hyperlipidemia patients with phlegm and blood stasis syndrome, indicated that heredity factor plays an important role.
