老年腹腔镜胆囊切除术的围术期处理

摘　　要：目的评价老年人腹腔镜胆囊切除术(LC)的安全性及可行性,探讨防范风险的应对措施。方法回顾性分析60岁以上老年人LC组(n=69)及中青年LC组(n=294)的临床资料,比较2组患者的术后转归。结果LC手术成功率100%,所有患者术后恢复良好,未发生胆管损伤等严重并发症,老年组与中青年组平均手术时间、术后住院时间、胃肠功能恢复时间、术后下床活动时间、肺部感染发生率、住院费用情况等均无统计学差异(P>0.05)。结论老年患者行腹腔镜胆囊切除术安全可行,掌握好适应证,做好围术期处理是老年腹腔镜胆囊切除术的要点。Objective To evaluate the safety and feasibility of laparoscopic cholecystectomy（ LC ） in elderly patients, and explore how to keep away its risk. Methods The clinical data of elderly patients whose age were over 60 treated by LC （ n=69 ） and the other adult patients whose age were under 60 treated by LC （ n=294 ） were analyzed retrospectively, and the recovery result of both groups were compared. Results The success rate of LC was 100%. Both of the two groups of LC were recovered smoothly with no serious complications such as bile duct injure, the mean operation time, the postoperative hospitalization time, the gastroenteric recovery time after operation, the infection rate of lung, the expense had no difference. Conclusion LC is a safe and feasible operation for aged. Strict indication and correct perioperative managements are the keys to LC in aged patients.
