运动中变时性对冠心病诊断价值的探讨

摘  要:目的:探讨运动中变时性对冠心病(CHD)的诊断价值。方法:测定分析90例患者,行平板运动试验(TET)及冠脉造影(CAG)。其中冠脉造影26例排除CHD,64例确定CHD,对比二组受检者在运动中是否存在变时性不良并与TET心肌缺血对比。结果:冠脉正常组运动后不易出现变时性不良(12%),而CHD组易出现变时性不良(78%),二组对比有显著差异,P〈0.01。其敏感性、特异性、准确性、阳性预测值均高于TET运动时ST压低指标。结论:运动中变时性不良是预测心肌缺血的间接标志,TET在诊断CHD测ST压低的同时,结合变时性不良更有意义,从而提高CHD早诊的阳性率和标准性。Objective: To investigate the diagnostic value of the chronotropic for CHDduring TET. Methods: 90cases of patients were tested and analyzed and each has completed TET and coronary artriography(CAG), 26cases of them wer excluded from the CHD group through CAG, The two groups of patients were compared on the chronotropic incompetence and compared with TET myocardial ischaemia. Results: Easy not showed the chro - notropic incompetence in the non - CHD group( 12% ) and easy showed the chronotropic incompetence in the CHD group(78% ). It showed very significant difference in both groups P 〈 0.01, They had higher sensitivity, specificit, accuracy, positive predictive value than those of ST - segment depression for detecting CHD. Conclusion: Chronotropic incompete - nce during exercise is indirect symbol of myocardial ischaemi- a. It is very important to meacure the chronotropic incompetence as well as ST during TET. It can promote the positive rate and accuracy rate of earlier diagnosis myocardial ischaemia for CHD patients.
