马来酸桂哌齐特与硫酸镁联合治疗外伤性蛛网膜下腔出血的疗效评价

摘　　要：目的：探讨马来酸桂哌齐特与硫酸镁联合应用治疗外伤性蛛网膜下腔出血（tSAH）的疗效。方法：240例tSAH患者随机分为对照组（60例）：给予止血、脱水、激素及促神经功能恢复药等常规治疗；治疗组Ⅰ（60例）：除常规治疗外，另给马来酸桂哌齐特160mg＋5％葡萄糖250m112h一次，用14d；治疗组Ⅱ：除常规治疗外，另给25％硫酸镁20ml＋5％GS250ml 12h1次，用14d；治疗组Ⅲ（60例）：治疗组Ⅰ、Ⅱ的措施联合应用。入院时及入院后第3d、7d、14d行颈颅多普勒（TCD）检查，观察蛛网膜下腔出血的并发症、脑血管痉挛（CVS）的发生情况及预后。结果：治疗组Ⅲ与对照组、治疗组Ⅰ、Ⅱ相比较，在CVS的发生、大脑中动脉血流速度的变化、3个月及6个月的预后方面并均有显著性差异（P均〈0．01）。结论：马来酸桂哌齐特与硫酸镁联合应用不仅能减轻tSAH并发生CVS的严重程度，而且能降低CVS的发生率和明显改善患者的预后。Objective： To study curative effect of cienpazide maleate combined with magnesium sulfate in treatment of traumatic subarachnoid hemorrhage（TSAH）. Methods：240 patients with TSAH were divided randomly into： Control Group（60 cases）, which was given routine therapy in- cluding hemostasis, dehydrating, hormone and nerve function restorative drugs. Treating Group I （60 cases）was givern, besides routine therapy, cienpazide maleate 160mg ＋ 5 % GS 250ml every 12h for 14 days. Treating Group Ⅱ （60 cases）was givern, besides routine therapy, 25 % magnesiun sulfate 20ml ＋ 5 % GS 250ml every 12h for 14 days, Treating Group Ⅲ （60 cases）was givern as Treating Group Ⅰ and Treating Group Ⅱ did. Trans- cranial Dopplar TCD examination was performec when admission snd the 3 - d, 7th and 14th day after admission. Occurence and prgnosis of conplication of subarachnoid henorr hage and cerebral vessel spasn（CVS） were observed. Results： Compared with control Group, Treating Group Ⅰ and Treating Group Ⅱ Treating Group Ⅲ had significat differences（all P 〈 0.01）. in occurrence of CVS, change of blood flow rate of middle cerebral artery and prognsis at 3 months and 6 months. Conclusion： Combination application of cienpazide maleate with magnesium sulfate can not only reduce CVS gravity but also lower incidence of CVS and markedly improve patient＇s prognosis.
