卷曲霉素及左氧氟沙星治疗复治涂阳肺结核疗效观察

摘　　要：目的观察和评价卷曲霉素及左氧氟沙星治疗方案对复治涂阳肺结核的作用和疗效。方法将98例抗酸杆菌阳性复治肺结核患者分成治疗组(n=58)和对照组(n=40),治疗组采用复治化疗方案加用卷曲霉素和左氧氟沙星,方案为3CVL2PaZ/9VL2Pa;对照组复治方案为3SEL2PaZ/9EL2Pa。结果治疗12个月后治疗组和对照组涂阳阴转率分别为82.76%(48/58)和60.00%(24/40),差异有显著性(P<0.05)。治疗组和对照组治疗有效率分别为93.10%(54/58)和70.00%(28/40),两组比较差异有非常显著性(p<0.01)。结论卷曲霉素和左氧氟沙星治疗复治涂阳肺结核方案有助于痰菌阴转和病灶吸收,可作为复治肺结核化疗的主要选择方案之一。Objectives To observe and evaluate the therapeutic effects of capreomycin and levofloxacin on retreatment smear positive pulmonary tuberculosis. Methods 98 cases of retreatment pulmonary tuberculosis with bacteriological positive were divided into treatment group （n=58） and control group （n=40）.The former group was treated by capreomycin , levofloxacin and other second-line antituberculosis drugs （3CVL2PaZ/gVL2Pa）, while the control group only incuded the same second-line antituberculosis drugs （3SEL2PaZ/9EL2Pa）. The treatment course was 12 months. Results After 12 months, the sputum negative conversion rates of treatment group and control group were 82.76%（48/58） and 60.00%（24/40） respectively, and the difference between the two groups was significant （p〈0.05）. Effective rates of treatment group and control group were 93.10%（54/58）and 70.00% （28/40）respectively （p〈0.01）. Conclusions Capreomycin and levofloxacin have a beneficial effect on treating retreatment smear positive pulmonary tuberculosis by improving cavity closure, sputum negative conversion and Xray manifestation.
