脑梗死患者高压氧治疗前后的血脂及血液流变学指标分析

摘　　要：目的:观察脑梗死患者血脂及血液流变学指标在高压氧(HBO)治疗前后的变化,探讨HBO治疗脑梗死疗效及机理。方法:将脑梗死患者(54例)随机分为HBO治疗(HBO组)和常规药物治疗(对照治疗组)两组,同时设立一健康对照组(60例)。所有对象均检测血浆甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[LP(a)]、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)水平及血液流变学相关指标,并作统计学分析。结果:脑梗死组与健康对照组比较,TC、TG、LDL-C、LP(a)明显升高(P<0.01),HDL-C明显降低(P<0.01),血液流变学6项指标均显著高于健康对照组(P均<0.01)。经治疗后,HBO治疗组血液流变学所有指标明显下降(P<0.01);血浆HDL-C浓度升高(P<0.05),其它血脂浓度无明显变化(P>0.05)。而对照治疗组变化不明显。结论:脑梗死患者存在明显高血脂及高血粘。HBO治疗可明显改善脑梗死患者血液流变性,降低血液粘度,但对血脂改善效果不明显。Objective: To investigate the changes of blood-lipid and hemorrheology in cerebral infarction with the treatment of hyperbaric oxygen(HBO),and to explore the curative effect and pathogenesis.Method: 54 cases of cerebral infarction patients were divided into hyperbaric oxygen (HBO) group and common drug treatment group. Set up normal blank group(60 cases) at the same time.All the cases were detected the triglyceride (TG),total cholesterol (TC),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein (LP-a),lipoprotein-cholesterol(LDL-C), lipoproteina(LP-a), apoprotein-AI(Apo-AI), apoprotein B (Apo-B), and hemorrheology before and after the treatment; All the datas were analysised with statistics.Results: In cerebral infarction group,the TC,TG,LDL-C,LP(a),hemorrheology were higher than the normal group(P<0.01), but HDL-C was lower than the control group obviously(P<0.01), ApoA1,ApoB were no difference(P>0.05); after HBO treatment, the parameters of hemorrheology were decreased noticeably(P<0.01), HDL-C was increased (P<0.05), there were no marked changes in other lipoid index. All the datas had no obviously change in the common drug treatment group.Conclusion: Hyperlipemia and high blood viscosity were always existed in cerebral infarction patients, the above parameters of hemorheology were changed significantly after received HBO,but the change of blood-lipid was not obviously.
