定量组织多普勒对糖尿病患者早期左室心功能的研究

摘　　要：目的应用定量组织多普勒技术评价糖尿病患者早期左室局部心肌的收缩及舒张功能变化。方法运用QTVI对40例正常人及42例糖尿病患者的左室中段各心肌节段的速度曲线进行分析,测量舒张期左室心肌的收缩期运动速度(Vs),舒张早期的运动速度(Ve),舒张晚期的运动速度(Va),并计算Ve/Va。常规超声心动图测量左室射血分数(LVEF)等。结果糖尿病组与正常对照组的LVEF等值均在正常范围,差异无统计学意义(P>0.05);糖尿病组舒张期室壁Vs低于正常组,Ve低于正常组,Ve/Va低于正常组,差异有统计学意义(P<0.001);Va高于正常对照组,差异无统计学意义。(P>0.05)。结论QTVI技术可定量评价糖尿病患者的早期左室心肌运动异常,表现为收缩及舒张功能减低,提示早期可能存在左室心肌缺血。[Objective] This study was designed to assess the left ventrlcular（LV）regional systolic and diastolic function and left ventricular wall motion features in patients with diabetes mellitus （DM）using qantitative tissue velocity imaging. [Methods] A total of 40 normal subjects and 42 DM patients underwent QTVI study. Offline LV regional velocity imaging and displacement profiles along LV apical longaxis view were obtained synchronously. Peak tissue velocities of LV regional muscular tissue during systole（Vs）, early diastole（Ve）, LA contraction （Va）, and calculate Ve/Va ratio. LV ejection fraction（LVEF） were measured by ultrasonic cardiogram. [Results] There was no statistical difference in LVEF between the patients with Dm and healthy subject（P 〉0.05）; The Vs, Ve and Ve/Va ratios in the patients with diabetes mellitus decreased, and there were statistical difference in these parameter between the patients with Dm and healthy subject （P 〈0.001）; There were no statistical difference in Va between the patients with Dm and healthy subject （P 〉0.05）. [Conclusions] QTVI could qantitatively assess the abnormal of the early left ventrieular function in DM patients, the systolic and diastolic function in DM patients were injured, so it could be inferred that there was subendocardial myocardial ischemia in patients with Dm.
