摘 要:目的:探讨子痫前期血清尿酸水平(UA)对预测母儿预后的影响。方法:测定77例子痫前期孕妇(UA正常31例,尿酸UA升高46例)及44例正常妊娠孕妇住院后的血尿酸值,观察两组孕妇血尿酸、肝、肾功能等生化指标的改变,分析子痫前期患者不同血尿酸值与母儿预后的关系。结果:子痫前期组UA、肌酐(Cr)、尿素氮(BUN)、谷丙转氨酸(ALT)、谷草转氨酸(AST)均高于正常妊娠组,而白蛋白(Alb)、白球比(A/G)、Ca2+均低于正常妊娠组,两组比较差异均有显著性(P<0.01)。尿酸升高组与正常组比较,Cr、BUN、Alb值有显著性差异(P<0.01),而ALT、AST、A/G、Ca2+差异无显著性(P>0.05);且产后出血、肾功能异常的发生率比正常组增加(P<0.01),而剖宫产、胎盘早剥、肝功能异常的发生率无显著性差异(P>0.05);尿酸升高组新生儿体重、Apgar评分明显低于正常组(P<0.01),而早产、FGR的发生率以及围产儿死亡率均高于正常组(P<0.05)。结论:尿酸升高与子痫前期及围产儿预后密切相关,可作为预测早期肾功能损害的敏感指标之一。Objective:To investigate the influence of serum uric acid to predict the pre-eclampsia and their perinatal outcome.Methods:The serum level of uric acid was measured in 77 cases with pre-eclampsia and 44 cases normal pregnancy in the hospital.The changes of the serum uric acid,liver and renal function in two groups,and the relation between different serum uric acid and the pre-eclampsia and their perinatal outcome were analyzed.Results:The levels of UA,Cr,BUN,ALT,AST,Mg^2+ in pre-eclampsia were all significantly higher in pre-eclampsia than that in normal pregnant women(P〈0.01),and the levels of Alb,A/G,Ca^2+ were all significantly lower than that in normal pregnant women(P〈0.01).In pre-eclampsia,there was significantly difference in the Cr、BUN、Alb among high serum uric acid group and normal serum uric acid group(P〈0.01),but there was no significantly difference in the ALT,AST,A/G,Ca^2+,Mg^2+(P〉0.05);The incidence of postpartum hemorrhage,abnormal renal function in high serum uric acid group was higher than that in normal serum uric acid group(P〈0.01),but the incidence of uterine-incision delivery,placental abruption and abnormal Liver function was no significantly difference(P〉0.05);The newborn weight,Apgar score in high serum uric acid group was lower than that in normal serum uric acid group(P〈0.01),and the incidence of premature labor,FGR and perinatal mortality rate in high serum uric acid group were all higher than that in normal serum uric acid group(P〈0.05).Conclusion:High serum uric acid level may be correlated closely with pre-eclampsia and perinatal outcome,and is the sensitive index to predict early abnormal renal function in pre-eclampsia.
