摘  要:目的分析广东地区高脂血症性急性胰腺炎(HLAP)的临床特征。方法收集广州市第一人民医院、韶关市粤北人民医院、汕头市中心医院和梅州市人民医院1990年至2005年急性胰腺炎(AP)病例共1582例。根据病因分为HLAP(123例)和急性胆源性胰腺炎(ABP)(828例)两组,比较两组患者在诱因、临床表现、实验室指标、疾病严重程度和预后等方面的差异。结果两组的临床症状和体征无显著差异;HLAP组的TG水平显著高于ABP组;但血清淀粉酶仅有36%的患者升高3倍以上,显著低于ABP组的62%。HLAP组的SAP比例、脏器衰竭发生率、总体病死率和复发率均显著高于ABP组(P<0.05)。结论HLAP具有显著的重症化和复发性趋势,预后较为凶险。Objective To analyze the clinical features of hypedipidemic acute pancreatitis (HLAP) in Guangdong province. Methods The medical records of 1,582 patients, who were admitted to Guangzhou First Municipal People's Hospital, Shaoguan Yuebei People's Hospital, Shantou Central Hospital and Meizhou People's Hospital with first attacks of acute pancreatitis and had their serum lipids examined during January 1990- June 2005, were reviewed. According to etiology, these patients were divided into HLAP group (123 cases) and acute biliary pancreatitis (ABP) group (828 cases). The cause, clinical manifestation, laboratory examination, severity and prognosis of the disease were compared. Results There was no significant difference in clinical symptoms and signs between two groups. The mean serum TG value in HLAP group was significantly higher than in ABP group, but serum amylase increase by 3 times in patients with HLAP was only 36% which was significantly lower than 62% in ABP group. The incidence of SAP, organ dysfunction, recurrence and mortality were significantly higher in HLAP group than in ABP group. Conclusion Patients with HLAP has remarkable severity and recurrent trend, and bad prognosis.
