摘  要:目的比较微创动力髋螺钉(MTDHS)和动力髋螺钉(DHS)治疗股骨颈基底部骨折的生物力学特性。方法将8具新鲜成人尸体股骨标本截骨,造成股骨颈基底部骨折,分别以MTDHS(实验组)和DHS(对照组)进行骨折固定,在实验机上测试两种器械的生物力学特性。结果MTDHS组在股骨头的位移变化、骨折固定强度和刚度、股骨头的扭转强度和刚度,股骨的极限力学性能等方面均明显优于DHS组(P〈0.01)。结论MTDHS治疗股骨颈基底部骨折比DHS更具生物力学优势,固定更加坚强,是当前一种理想的内固定器械。Objective To compare biomechanics of micro-trauma dynamic hip screw (MTDHS) and dynamic hip screw (DHS) in treatment of fracture of femoral neck basement. Methods Femoral neck basement fractures of eight cadaveric femur caused by a saw were treated with MTDHS ( MTDHS group) and DHS (DHS group). Biomechanical characteristics of two different devices were compared. Results MTDHS group was better than DHS in regard of dislocation of femoral head, strength and rigid of fracture fixation, twist force and strength as well as limited mechanics ( P 〈 0.01 ). Conclusions Experimental results show that MTDHS offers distinct biomechanical adventage over DHS in treatment of fracture of femoral neck basement and is an ideal internal fixation instrument.
