甲状腺功能亢进症介入栓塞治疗的临床研究

摘　　要：目的探讨介入栓塞治疗甲状腺功能亢进症的方法并分析临床疗效。方法对15例甲状腺功能亢进症患者行甲状腺动脉造影后,根据造影结果用聚乙烯醇微球(PVA)栓塞相应的甲状腺动脉。结果介入栓塞结果如下:①左、右双侧甲状腺上动脉5例;②左、右双侧甲状腺上动脉+左侧甲状腺下动脉5例;③左右双侧甲状腺上动脉+右侧甲状腺下动脉3例;④左侧甲状腺上动脉+右侧甲状腺下动脉1例;⑤左右双侧甲状腺下动脉1例。随访12个月,TT3自(6.20±3.06)mmol/L降至(3.07±1.14)mmol/L(P<0.05),TT4自(198.30±78.54)mmol/L降至(143.05±33.70)mmol/L(P<0.05);甲状腺明显缩小,临床症状消失或缓解,未见严重的并发症。停药10例,需少量抗甲状腺药物维持5例。结论甲状腺动脉栓塞是治疗甲状腺功能亢进的有效方法,应根据甲状腺动脉造影的血供及甲状腺染色大小来确定治疗过程中需要栓塞的甲状腺动脉数目。Objective To investigate the methods and clinical effects of thyroid interventional embolization. Methods Thyroid artery of patients with Hyperthyroidism were arteriographied, then selectively embolized with polyvinyl alcohol （PVA）. Results The interventional embolization was done as below：（1）five patients：.right and left superior thyroid arteries embolized. （2）five patients：right and left superior ＋ left inferior thyroid arteries embolized; （3） three patients ： right and left superior＋right inferior thyroid arteries embolized;（4）one patient：left superior ＋right inferior thyroid arteries embolized;（5）one patient .right and left inferior thyroid arteries embolized. Followed up 12 months,TT3 dropped from （6.20±3.06）mmol/L to （3.07±1.14）mmol/L （P〈 0.05） ,TT4 dropped from （198. 30±78.54）mmol/L to （143.05±33.70）mmol/L（P〈0.05） ;The size of thyroid decreased remarkable and the symptoms of hyperthyroidism were controlled and no serious complications were found. Drug administration was discontinued in 10 patients and just a small dose for 5 patients. Conclusion Interventional therapy for hyperthyroidism is an effective method,the number of thyroid arteries to be embolized depended on the blood supply from arteriography and the density of arteriography.
