应用缝合锚钉急诊修复四肢腱性组织止点区断裂伤的研究

摘　　要：目的探讨急诊应用缝合锚钉修复四肢肌腱和韧带组织的可行性、手术技巧及临床疗效。方法2003年8月～2006年6月采用缝合锚钉急诊一期修复四肢肌腱及韧带等腱性组织止点附近断裂伤患者41例，术后第2天即开始严格指导下康复训练，直至完全康复。从创口愈合、关节活动范围和肢体运动功能状态、工作和生活恢复、放射学检查、局部疼痛、修复组织再断裂、异位骨化等并发症发生率方面观察腱性组织愈合及肢体功能恢复情况。结果41例患者中36例获得5—24个月（平均9．3个月）随访。所有患者创口均一期愈合，关节稳定性好，关节运动范围及肢体运动功能状态恢复：优（90％以上）14例，好（80％～90％）19例，满意（70％～80％）3例；工作及生活满意，X线片示锚钉在骨内无移位，除2例残留局部轻微疼痛外，无肌腱及韧带再断裂发生。结论应用缝合锚钉急诊修复四肢腱性组织止点区断裂伤，有利于腱性组织的早期愈合，手术简捷、创伤小、固定确实、力学性能好、可早期康复锻炼、并发症发生率低，是一种可靠而有效的治疗方法。Objective To study the techniques and curative effects of repairing terminal ruptures of tendons and ligaments at the extremities with suture anchors in emergency. Methods From August 2003 to June 2006, 41 cases of the terminal ruptures of tendons and ligaments at extremities were repaired with suture anchors in emergency. The next day after operation, regular rehabilitation was conducted for every patient until complete recovery. The healing of tendon tissues and functional recovery were observed with respect to wound healing, joint motion range, joint stability, effect on work and life, radiographic findings, and complications. Results Thirty-six of the 41 cases were followed up for 5 to 24 months （average, 9.3 months） . Primary healing of wounds was observed in all cases. Compared with normal opposite extremities, the joint motion range and functional recovery of extremities were rated as excellent in 14 patients （over 90% ）, good in 19 patients （over 80% ）, satisfactory in 3 patients （over 70% ）. All patients were satisfied with the effects on their current work and life. No displacement of anchors was found in radiographic films and no such complications as re-rupture and joint stiffness were observed except in 2 patients who experienced remaining vague pain and in 1 patient who developed heterotopic bone formation. Conclusion Repairing ruptures of tendons and ligaments at the extremities with suture anchors in emergency can accelerate healing of injured tendons and ligaments, secure rigid fixation, cause slight invasion and low complications, allow early rehabilitation, and provide fine biomechanic performance.
