原发性夜间遗尿症儿童行为问题及相关因素研究

摘　　要：目的探讨原发性夜间遗尿症（PNE）儿童的行为问题及相关因素,为早期心理干预提供依据。方法采用Achenbach儿童行为量表（CBCL）对36例单纯PNE儿童、26例PNE＋注意缺陷多动（ADHD）儿童和36例正常儿童进行对照研究,并对其行为问题的影响因素进行分析。结果各组儿童行为问题检出率分别为：单纯PNE儿童36.11%、PNE＋ADHD组儿童45.15%,正常儿童11.11%。各年龄段儿童行为问题检出率分别为5～8岁13.33%、9～12岁25.00%,13～17岁33.33%。CBCL量表各因子：单纯PNE组和对照组比较,抑郁、交往不良、社交退缩,行为问题总分差异有显著性（P〈0.05）;单纯PNE组和PNE＋ADHD组比较,多动、违纪,行为问题总分差异有显著性（P〈0.05）。相关因素分析显示影响PNE儿童行为问题最主要的因素为年龄,父母文化程度和家庭收入。结论PNE儿童存在行为学异常,伴有ADHD时更为严重;年龄越大行为问题的检出率越高提示认为行为问题随年龄增大而自然消失是不正确的;父母文化程度及家庭收入越高,行为问题检出率越低。Objective To investigate the behavioral problem and associated factor in children with primary nocturnal enuresis（PNE） and provide basis for forepart psychology intervene. Methods Applying achenbach children behavior checklist （CBCL）, 36 PNE children, 26 PNE with attention-deficit hyperactivity disorder （ADHD） children,36 ADHD children, 36 normal children were compared and the associated factors were analyzed. Results The incidence on behavioral problems in PNE group, PNE ＋ ADHD group and normal group were 36.11%, 45.15% and 11.11% respectively. In different age group：13. 33% for5 -8 years old,25.00% for9 - 12 years old, 33.33% for 13- 17 years old. Depression, dyscommunication, social intercourse recession and behavioral problem score had statistic significant between PNE group and control group（ P 〈 0.05 ） ; and hyperactivity disorder, delinquence and behavioral problem score had statistic significant between PNE group and PNE ＋ ADHD group （ P 〈 0.05 ）. Associated factor analysis showed age, parents culture degree and family income was the factors to influence behavioral problem. Conclusion PNE had behavioral problem which was aggravated by ADHD. it was wrong for the view that the behavioral problem will disappear with the increase of age, and they should do psychology intervene early. The higher the culture degree and family income, the lower the incidence on behavioral problem, which indicate that behavioral problem was related to the socieconology status of their parents.
