制霉菌素鱼肝油配伍已烯雌酚用于老年妇女阴式手术的术前准备

摘　　要：目的:探讨绝经后妇女阴式手术前行阴道准备的合理用药。方法:Ⅱ度或Ⅲ度子宫脱垂伴不同程度阴道前后壁膨出的绝经后妇女80例,按1∶1随机入组,研究组每日两次阴道上药(制霉菌素鱼肝油配伍已烯雌酚),对照组每日两次行鸭蛋清阴道上药,对比分析两组用药后患者的阴道健康评分(VHS)、阴道细胞成熟指数(MI)、致密核指数(KI)、术前阴道上药所需时间、术中出血量、手术时间、术后不良反应等指标并分别测定两组用药前后血清E2、FSH水平变化,综合评价患者的疗效及安全性。结果:研究组的VHS、MI、KI与对照组相比明显升高(P<0.05);术中出血量、手术时间与对照组相比明显降低(P<0.05);上药后血清E2、FSH水平变化不明显(P>0.05)。结论:制霉菌素鱼肝油配伍已烯雌酚阴道上药是绝经后妇女阴道手术前行阴道准备的较好用药选择。Objective：To approach into choosing the suitable drug for preserved vigina before colpohysterorrhaphy in postclimacteric patients. Methods： Eighty postclimacteric women, with Ⅱ or Ⅲ degree hysteroptosis following different knuckle from the anterior and posterior viginal wall, were randomly divided into experiment and control groups. Nystatin-oleum morrhuae with the compatibility of diaethylstilbestrol was applied to painting on the viginal wall of the experimental group, and pale blue was applied to the controls, both of them with 2 times/day. Two groups were compared and evaluated for their viginal health scores（VHS）, maturation indexes（MI）of viginal exfoliative squamous cells, karyopyknotic indexes（KI）, medication periods of preoperational, intraoperative bleeding amounts,operation time,postoperative adverse reactions,levels of serum E2 and FSH etc. respectively. Results：The results of VHS, MI,KI were obviously increased and intraoperative bleeding amount,operation time of the experiment group decreased compared with those of the control group, which showed significantly different（P〈0.05）respectively, levels of serum E2 and FSH both the groups were not statistical significanee（P〉0.05）. Conclusion： Nystatin-oleum morrhuae with the compatibility of diaethylstilbestrol is the best drug applying for preserved vigina before colpohysterorrhaphy in postclimacterie patients.
