连续血液滤过治疗严重烧伤并发脓毒症患者32例

摘　　要：目的连续性血液滤过治疗严重烧伤并发脓毒症的疗效观察。方法32例严重烧伤并发脓毒症患者进行连续性血液滤过治疗,血滤前后观察生命体征,有创监测血流动力学的变化,检测电解质、肾功、血小板计数及血凝四项、血气、C-反应蛋白(CRP)、肿瘤坏死因子(TNF-α)、白介素-6 (IL-6)等。结果病人生命体征明显改善。血滤前后检测结果用均数±标准差表示,各计量指标资料采用t检验P<0.01具有统计学意义。显示出良好的效果。32例病人,死亡3例,其余治愈,治愈率达90.9%。结论在脓毒血症发生的早期,连续性血液滤过是除了药物与手术之外,通过体外循环途径去降低各种炎症介质,可作为烧伤脓毒症防治的另一条有效治疗手段。Objective To evaluate the therapeutic efficacy of continuous venous-veno hemofihration （CVVH） in treating severe burn patients with sepsis. Methods 32 cases of severe burn patients with sepsis had received CVVH treatment. Arterial blood gas analysis was done and Vital sign and haemodynamics were monitored, Vein blood was gotten before and after CVVH treatment and the contents of electrolytes, blood platelets count,BUN, SCr , were measured by normal method and C- reactive protein（CRP） ,TNF-α, IL-6 by EL ISA method. Result The outcome of before CVVH and after CVVH was significantly different （P 〈 0.01 ） . In the 32 cases, 29 cases were cured, 3 cases died. Conclusions Hemopurgation has decisively effect on the earlier period of sepsis. It could delete inflammation medium and poison , maintain the stability of the inside circulation and fill up nutrition , which is an effective treatment method to burn sepsis.
