摘 要:目的探讨中小面积热力烧伤病人感染期微循环改变及其特点。方法用WX-9B型及WX-753B型微循环显微镜及微循环图像分析系统,观测56例处于感染期的中小面积热力烧伤病人(烧伤面积19.2%±8.2%)足甲襞微循环(food nailfold microcirculation,FNM)变化,并与同样数量健康人比较。观测指标包括微血管形态、微血流动态、微血管周围状态三方面共十六项指标。结果中小面积烧伤病人FNM在感染期存在一定程度的紊乱,主要表现为管襻缩短、管径变细、微血流速度减慢、红细胞聚集存在、血液灌流量减少、襻周有渗出等。结论中小面积烧伤可引起全身微循环改变,治疗中小面积烧伤时可采用改善微循环疗法。Objective To explore the skin microcirculation changes of mild and moderate burned patients during infection period. Methods Fifty-six patients in infection period (average burned area: 19. 2%±8.2% ) were recruited in the study. The foot nail-fold (FNM)capillary shape and a toatal of 16 indicators manifesting the status of capillary vessels as well as the surroundings were observed by means of poly- position microcirculatory microscope and videotaped image analyses system (WX-9B). These findings were campared with 56 healthy adults. Results The FNM disorder occurred during the infection period of mild and moderate burned patients, and the major changes observed were capillary loops deeurtation, caliber thinning, induction of blood flow rate, the RBC aggregation and the decreased blood perfusion as well as the exudation of capillary, etc. Conclusions The body microcireulation disorder occurred in the and moderate burned patients. It is important to improve the microeirculation for these patients.
