慢性乙型肝炎患者外周血淋巴及单核细胞表面B7-H1表达研究

摘　　要：目的探讨B7家族共刺激分子B7-H1在慢性乙型病毒性肝炎(乙肝)患者外周血淋巴细胞及单核细胞表面的表达情况。方法利用荧光抗体标记结合多色流式技术,检测慢性乙肝患者外周血T淋巴细胞、B淋巴细胞、单核细胞群体表面B7-H1分子的表达。结果在慢性乙肝患者,T、B淋巴细胞表面B7-H1低表达,与健康对照无差异;单核细胞表面B7-H1的表达率为(19.17±11.64)%,显著高于健康对照的(7.30±5.49)%(P<0.01),且B7-H1的表达与患者血浆ALT水平成正相关(r=0.423,P<0.01)。结论慢性乙肝患者外周血T、B淋巴细胞表面B7-H1低表达,单核细胞B7-H1表达上调,且与ALT水平正相关,提示B7-H1可能与乙肝病毒感染慢性化相关。Objective To investigate the expression of B7-H1 on peripheral blood mononuclear cells from patients with chronic HBV infection. Methods Immunofluorescent staining and flow cytometry assay were used to measure the expression of B7-H1 on peripheral blood CD3^high T cells, CD19^high B cells and CD14^high monocytes from chronic HBV infected patients. Results No significant difference was observed in B7-H1 expression on T cells and B cells between chronic HBV infected patients （CHB） and health controls （HC）. B7-H1-expressing CD14^high cells were significantly increased in chronic HBV-infected patients （19.17±11.64）% as compared with healthy controls [（7.30±5.49）%, P〈0.01）. A significant positive correlation was found between B7-H1 expression on CD14^high monocytes and serum ALT levels. Conclusion There is no significant difference in B7-H1 expression on T cells and B cells between CHB patients and healthy subjects. B7-H1, which is up-regulated on monocytes from chronic HBV-infected patients, in positively correlated to serttm ALT levels, and may play a role in the persistence of HBV infection.
