摘 要:目的:为了观察软骨缺损的修复过程,比较不同数目钻孔术对软骨缺损的修复效果。方法:用中国白兔24只,在股骨关节面造成6mm×8mm全层软骨缺损,分别施行10孔及5孔钻孔术,术后4、8周取材,做组织学及电镜观察,并进行评估。结果:(1)10孔、5孔和对照组的优势修复组织分别以类透明软骨,类透明软骨加纤维软骨和纤维组织为主。(2)修复组织厚度10孔及5孔无显著差异。(3)修复组织覆盖缺损的面积,10孔>5孔>对照组。初步结论:软骨下骨钻孔可修复关节软骨全层缺损;多孔比少孔修复好;非钻孔的缺损修复效果较差。In order to observe the procees of repairing articular cartilage defect and compare the effect of repairing after subchondral drilling with different holes. Methods: Full thickness defects of 6mm×8mm were created over the articular suface of both the femoral condyles in 24 rabbits. The 10 and 5 holes were drilled respectively over the defects with the exception of those of the control ones. They were sacrificed postoperatively at the end of 4 and 8 weeks. The newly formed tissue in the defects were examined histologically and ultrastructurally. The effect of repair defect in different groups was compared and evaluated. Results: Hyalin like cartilage was formed in the 10 holes group, hyalin like caetilage or fibrous cartilage was identifid in the 5 holes one. and the fibrous tissues was found in the controls. The thickness of repair tissues had no significant difference between the 10 holes and 5 holes group. The coreved areas of repair tissue of the 10 holes group exceeded those of 5 holes and control ones. Preliminary concluded: Subchondral bone drilling is feasible for repairing full thickness articular cartilage defects. The repair effect of 10 holes is superior to that of 5 holes. When the defects are not drilled, the tissue regeneration is much less and the repair in sufficient.
