定坤育子汤配合针灸促卵泡发育的临床研究

摘　　要：目的:探讨补肾中药定坤育子汤配合针灸对卵泡发育的影响、该影响与妊娠率的关系以及针灸促排卵机制。方法:将61例中医肾虚型卵泡发育障碍患者随机分为针药组、中药组、西药组,3组病人数分别为22、19、20,第1组于月经第5天开始给予中药,至第12天加用针灸,至卵泡发育成熟排出时停止;中药组于月经第5天开始给予中药,至卵泡发育成熟排出时停药;西药组于月经第5天开始服用克罗米芬50mg,每日1次,共服5天。所有病例从月经第10天始行B超监测卵泡直径、子宫内膜厚度直至卵泡排出。结果:各组卵泡直径治疗前后相比较均有非常显著性差异(P<0.01),针药组、中药组子宫内膜厚度治疗前后比较无显著性差异(P>0.05),西药组治疗前后相比较有显著性差异;疗效组间比较,针药组与中药组皆使内膜增厚,两者比较无显著性差异(P>0.05),西药组治疗后内膜较治疗前薄(P<0.05),针药组与西药组比较有显著性差异(P<0.05),中药组与西药组比较有显著性差异(P<0.05)。结论:补肾中药定坤育子汤通过促进卵泡发育,针灸及时使卵泡破溃,诱发排卵,从而提高妊娠率。Objective： To investigate the influence of the Kidney - Nourishing Traditional Chinese medicine dingkun yuzi decoction cooperating with acupuncture and moxibustion on follicular development;relationship between the influence and increasing pregnancy rates and the mechanism of acupuncture and moxibustion stimulating ovulation. Method： Sixty - one infertile women with follicular maldevelopment and kidney Qi insufficiency were divided into 3 groups at random：Medicine Needle group,Traditional Chinese Medicine group,Western Medicine group. The three group had 22,19,20 patients respectively. The group 1 and group 2 were given Kidney Nourishing Traditional Chinese medicine on day 5 of the menstruation until follicle mature and rupture, Group 3 were given 50mg Clomiphene Citrate （CC） on the day 5 of menstruation , for 5 days. Each patient ＇s dominant follicle diameter and endometrium thickness were observed by using vagina B ultrasonic wave since the day 10 of menstrual cycle until ovulation. Result： Following treatment, the dominate follicle＇s diameter significantly increased in size （ P 〈 0.01 ）. The difference between groups are not significant（ P 〉 0.05 ） ;the endometrium of the group 3 was significantly thinner than that before treatment（ P 〈 0.05 ）. Curative effect group comparison：There is significant difference between group 1 and group 3（ P 〈 0.05） ,So does Traditional Chinese medicine group （ P 〈 0.05 ）. Conclusion ： Nourishing kidney traditional Chinese medicine promotes follicular development;acupuncture and moxibustion stimulate follicle mature and ovulation,thus increase the pregnancy rate.
