脑室积血外引流闭式冲洗与常规开放冲洗的比较

摘  要:目的探寻一种简单、有效、安全的脑室积血外引流冲洗治疗方法。方法选择30例高血压性脑出血脑室积血伴脑积水的患者,随机分成两组,15例脑室闭式冲洗引流,另15例开放冲洗引流。结果脑室积血外引流闭式冲洗与开放冲洗比较:(1)冲洗实际操作时间:前者平均(7.13±1.68)min明显低于后者平均(42.33±8.58)min,差异有统计学意义(P<0.000 1);(2)更换头部敷料次数:前者平均(5.93±1.33)次明显低于后者平均(13.27±2.55)次,差异有统计学意义(P<0.0001);(3)前者一人独立完成,而后者需他人协助;(4)前者冲洗液不与换药碗、空气接触,后者则是开放的;(5)冲洗引流效果:前者与后者一致,差异无统计学意义(P>0.05)。结论脑室积血外引流闭式冲洗优于开放冲洗,是一种可推荐采用的简单、有效、安全的脑室积血外引流冲洗治疗方法。Objective To explore an irrigation method of simpleness, availability and security in external ventricular drainage for intraventricular haemorrhage. Methods Thirty patients with external ventricular drainage due to hypertension intraventricular haemorrhage were randomized to two treatment groups. Fifteen patients were enrolled in the close irrigation group and another fifteen cases were enrolled in the open irrigation group. Results Comparing the close irregation with the open irregation: (1)the actual time of operation:the fomerV(7.13+1.68)mini was lower than the latterV(42. 33+ 8. 58)min](P〈0. 000 1), (2)the times of changing dressing at head:the formerV(5.93+1.33)times] was lower than the latter[(13.27+2.55)times](P〈0. 000 1), (3)the former by one doctor and the latter finished the irrigation by two doctors, (4)the water of irrigation: the former was close and the latter was open, (5)the therapeutical outcomes of external ventricular drainage in two groups was identical(P〉0.05). Conclution The close irrigation of external ventricular drainage due to intraventricular haemorrhage is a better method than the open irrigation. It is an irrigation method of simpleness, availability and security.
