摘 要:目的探讨睫状神经营养因子(CNTF)与七叶皂苷钠(SE)联合应用对脊髓牵拉损伤的保护作用。方法72只大鼠随机分为模型对照组、CNTF治疗组及CNTF+SE治疗组,牵拉致伤T10-2脊髓建立脊髓牵拉损伤模型。CNTF组经尾静脉注入CNTF20μl;CNTF+SE组除同法应用CNTF外,术后次日始腹腔内注射七叶皂苷钠1.5mg·kg^-1·d^-1,连续应用7d;模型对照组以相同方法给予等量生理盐水。伤后1d、3d、7d、14d进行BBB行为学评分,每组6只,然后处死动物取脊髓分别作HE染色、免疫组化及TUNEL,观察神经细胞结构的变化,并检测bcl-2、Bax阳性细胞及凋亡细胞。结果(1)bcl-2:实验组7d达高峰,CNTF组为(44.27±5.93)%,CNTF+SE组(46.26±6.01)%,模型对照组为(27.46±5.65)%,差异有统计学意义(P〈0.05);(2)bax:实验组3d达高峰,CNTF组为(39.15±5.91)%,CNTF+SE组(32.63±5.34)%,模型对照组为(46.69±7.52)%,差异有统计学意义(P〈0.05)。(3)各时间点治疗组细胞凋亡指数较模型对照组低。结论睫状神经营养因子与七叶皂苷钠对大鼠脊髓牵拉造成的损伤有保护作用,二者联合应用具有协同作用。Objective To explore the protective effects of ciliary neurotrophic factor (CNTF) in combination with sodium escine(SE)on spinal cord injury (SCI) in rats and its mechanism. Methods Seventy-two Wistar rats were randomly divided into control group, CNTF group and CNTF+ SE group. The spinal cord at T10-12 level was pulled horizontally for 1.7 mm, with duration of 5 minutes, then the rats model was established, The locomotion function of hind limes of rats was evaluated by using the Basso, Beattie and Bresnahan (BBB) locomotor rating scale, The rat models were sacrificed at 1, 3, 7, 14 d (n=6 at each time point). A series of 5 μm cross section were cut at epicenter and caudal to epicenter, The Bcl-2 and Bax protein expressions were detected by immunochemistry, and terminal deoxynucleotidyl transferase medicated d-UTP nick end labeling (TUNEL) for cell apoptosis were conducted in three groups. Results (1)At 7 d, Bcl-2 expression reached its peak in CNTF group of (44, 27 ± 5.93)%, CNTF + SE group of (46, 26±6.01 )%, as compared with (27.46±5, 65)% in the control group, with statistical differrence(P〈0. 05), (2)At 3 d, expression of bax reached its peak in CNTF group of(39. 15±5.91)% ,CNTF+SE group of(32.63±5.34)%, as compared with (46.69±7.52)% in the control group, with statistical differrence(all P〈0.05). (3) At each time point, the apoptosis index in the experimental group was lower than in the control group, Conclusions Ciliary neurotrophic factor and sodium escine could protect the neurological function in tractive SCI and combination of them has synergistic effects
