摘 要:目的探讨糖代谢异常孕妇中胰岛素抵抗(IR)和胰岛β细胞功能的关系,以及胰岛素抵抗程度与巨大儿发生的关系。方法测定35例妊娠期糖耐量异常孕妇(GIGT)和12例妊娠期糖尿病孕妇(GDM)的空腹血糖、胰岛素,采用稳态模型评估法(HOMA)计算GIGT和GDM的胰岛素抵抗指数(HOMA-IR)和胰岛β细胞功能指数(HBCI)。结果GIGT、GDM两组孕妇HBCI无统计学差异(P>0.05),而HOMA-IR差异有统计学意义(P<0.05);HOMA-IR与巨大胎儿的发生没有显著相关性。结论GDM较GIGT孕妇存在更为明显的胰岛素抵抗,没有发现胰岛素抵抗指数与巨大儿的发生相关。Objective To study insulin resistance and islet fl cell function in women with gestational impaired glucose tolerance and diabetes mellitus, and to investigate the relationship between insulin resistance and macrosomia. Methods In 35 GIGT women and 12 GDM women, fasting blood glucose and insulin levels were measured, insulin resistance index (HOMA- IR) and βcell function index (HBCI) calculated by HOMA model. Results there was no difference in HBCI in the two groups, but significant difference in HOMA - IR; HOMA IR in the GDM women was higher than in the GIGT group. There was no significant correlation between HOMA - IR and macrosomia. Conclusion Women with GDM have stronger IR compared with GIGT women, and no significant difference was found between HOMA - R and macrosomia.
