妇产科门诊异常宫颈涂片的管理及3233例分析

摘　　要：目的建立并完善妇产科门诊异常宫颈细胞涂片管理制度。方法对2005年1月1日至2006年12月31日在北京大学第三医院妇产科门诊行宫颈液基细胞学涂片的患者中,结果为不能明确意义的不典型鳞状细胞及以上患者3233例,依据美国阴道镜与宫颈病理协会推荐的异常宫颈涂片处理指南进行处理,并行回顾性分析。结果3233例异常宫颈涂片患者中行阴道镜检查1275例,其中67.6%的高危型HPVDNA阳性的不能明确意义的不典型鳞状细胞、68.9%的不能除外高度鳞状上皮内病变的不典型鳞状细胞、72.9%的低度鳞状上皮内病变、80.2%的高度鳞状上皮内病变患者行阴道镜检查,通过加强对异常宫颈涂片的管理,2006年比2005年有更高比例的宫颈涂片患者在本院行阴道镜检查,共检出宫颈上皮内瘤变2级及以上病变(CIN2+)243例。结论加强异常宫颈细胞学涂片的管理在宫颈防癌筛查工作中有重要意义。Objective To establish and enhance the supervising system for abnormal cervical cytology in gynaecological outpatients. Methods Total 3 233 patients in department of Gynaeeology and Obstetrics, Peking University Third Hospital. with abnormal cervical liquid - based cytology were included in this retrospective study from January 1^st 2005 to December 31^st 2006, The patients＇ management was according to the guidelines of American Soci ety of Colposeopy ＆ Cervical Pathology. Results Of the 3 233 cases, 1 275 patients consulted to colposcopist. Of the cases with atypical squamous cells undetermined significance （ASC - US） with detected high - risk HPV DNA, atypical squamous cells cannot exclude high grade squamous intraepithelial lesion （ASC- H）, low squamous intraepithelial lesion （LSIL） and high grade squamous intraepithelial lesion （HSIL）. 67.6% , 68. 9% , 72. 9% and 80. 2% visited colposcopist, respectively. More cases visited the colposeopist in our hospilal in 2006 than that in 2005, Among the 1 275 cases. 243 cases of cervical intraepithelial neoplasia grade 2 or more （CIN 2^＋ ） were detected. Conclusions It is important to enhance the supervising system for abnormal cervical cytology in cervical cancer screening.
