摘 要:目的探讨生长分化因子-9(growth differentiation factor-9,GDF-9)和骨形态发生蛋白-15(bone morphogenetic protein15,BMP-15)在卵母细胞和卵冠丘颗粒细胞(cumulus granulosa cell,CGC)的表达与人卵巢反应性的关系。方法应用免疫荧光结合激光共聚焦显微术检测31例卵巢中反应患者的34枚、23例高反应患者的29枚未成熟生发泡(germinal vesicle,GV)期卵母细胞GDF-9蛋白的表达,33例中反应患者的37枚、29例高反应患者的34枚第一次减数分裂中期(metaphase I,MI)的卵母细胞BMP-15蛋白的表达;应用免疫细胞化学方法检测两种蛋白在34例低反应、41例中反应和36例高反应患者第2次减数分裂中期(MII)CGC的表达。结果卵巢中、高反应组间卵母细胞GDF-9的表达差异无显著性,卵巢高反应组"卵母细胞"BMP-15表达强度为(39.19±8.84)显著高于中反应组(29.23±7.83,P<0.01),并与获卵数中度正相关,(r=0.492,P=0.002);卵巢不同反应组间CGC中GDF-9和BMP-15蛋白表达强度无统计学意义,但两种因子的表达呈中度正相关关系,(r=0.415,P=0.000)。结论卵巢高反应与卵母细胞BMP-15的表达升高有关;GDF-9和BMP-15共表达于CGC中,且具有同步性,并可能存在协同作用。Objective To determine the relationship between the expression of GDF- 9 and BMP - 15 proteins in oocyte and cumulus granulosa cell (CGC) and different ovarian response. Methods Both germinal vesicle (GV) stage and metaphase I (MI) stage oocytes which were not suitable for intracytoplasmic sperm injection procedure from patients with normal and hyper response, who undergo in vitro fertilization treatment in our center. Using immunofluorescence and laser scanning confocal microscope, the protein expression of GDF - 9 on GV - stage oocytes for 31 normal responders (34 oocytes) and 23 hyper- responders (29 oocytes) were analyzed. The expression of BMP -15 protein on MI stage oocytes for 33 normal responders (37 oocytes) and 29 hyper- responders (34 oocytes) also were examined, Using by immunocytochemistry, the protein expression of these two factors in MI- stage CGC of 34 poor responders, 41 normal responders and 36 hyper-responders were detected, respectively. Results Both of GDF- 9 and BMP - 15 evenly expressed in human oocyte cytoplasma. The expression of GDF - 9 has shown no statistic difference among different responders. While the expressions of BMP - 15 in oocytes for hyper - responders (39.19±8. 84) were higher than that of poor responders (29.23±7.83), and were positively correlated to the number of oocytes retrieved, (r=0. 492, P=0. 002). Both of GDF - 9 and BMP - 15 were expressed in CGC cytoplasma, however, they do not demonstrate statistic differences among different responders. But the expression of GDF- 9 were closely related with BMP- 15 in CGC, (r= 0. 415, P=0. 000). Conclusions The expression of BMP - 15 protein in oocyte but not GDF - 9 is closely related to hyper - ovarian response. Both GDF - 9 and BMP - 15 expressed synchronically in CGC.
