摘 要:目的探讨 Ras 相关结构域家族基因1A(RASSF1A)启动子区异常甲基化与食管癌发生发展的关系。方法采用甲基化特异性 PCR 方法,检测66例食管癌组织及相应的癌旁正常组织RASSF1A 基因启动子区异常甲基化。分析食管癌组织中 RASSF1A 基因启动子区异常甲基化与临床病理特征的关系。结果在食管癌组织、癌旁正常组织中 RASSF1A 基因启动子区异常甲基化的发生率分别为48.5%(32/66)、6.1%(4/66)。淋巴结转移者食管癌组织中 RASSF1A 基因启动子区异常甲基化的发生率(61.1%)明显高于无淋巴结转移者(33.3%)(x^2=5.055,P=0.025);晚期(Ⅲ-Ⅳ期)食管癌组织中 RASSF1A 基因启动子区异常甲基化的发生率(69.2%)明显高于早期(Ⅰ~Ⅱ期)(35.0%)(x^2=7.392,P=0.007);食管癌组织中 RASSF1A 基因启动子区异常甲基化与肿瘤分化程度无相关性(P>0.05)。结论食管癌组织中存在 RASSF1A 基因启动子区的异常甲基化,甲基化与食管癌病理分期和淋巴结转移有关。Objective To investigate the hypermethylation of the promoter region of RAS assoeiation domain family genelA (RASSF1A) and its relationship with esophageal squamous eell eareinoma. Methods Sixty-six patients with esophageal squamous eareinoma, 60 males and 6 females, aged 59 ±8 (44- 76 ), underwent reseetion of the tumor. Methylation-speeifie PCR (MSP) was used to deteet the hypermethylation of promoter region of RASSF1A in the eareinoma tissues and the adjaeent normal tissues. Results The hypermethylation rate of RASSF1A promoter in the tumor tissues was 48. 5% (32/ 66), signifieantly higher than that in the adjacent normal tissues ( 6. 1%, 4/66, P 〈 0. 05 ). The hypermethylation rate of RASSF1A promoter of the patients with lymph node metastasis was 61. 1% , signifieantly higher than that of the patients without lymph node metastasis ( 33.3%, χ^2 = 5. 055, P = 0. 025 ). The hypermethylation rate of RASSF1A promoter in the esophageal squamous eell eareinoma at advaneed stages ( stages Ⅲ- Ⅳ ) was 69.2% , signifieantly higher than that in the esophageal squamous eell eareinoma at early stages ( stages Ⅰ - Ⅱ, 35.0% , χ^2 = 7. 392, P = 0. 007 ). The hypermethylation rates of RASSF1A promoter in the high, moderate, and low differentiation tumors were 61.5% (16/26) , 46.2% (12/26), and 28.6% (4/14)respeetively without signifieant differenees among them (χ^2 = 4. 053, P =0. 132). Conclusion Abnormal methylation exists in the RASSF1A promoter in esophageal squamous eell eareinoma. The hypermethylation of RASSF1A promoter is related to lymph node metastasis and TNM stage.
