婴儿肺切除手术24例分析

摘　　要：目的:探讨婴儿肺切除手术的病因、手术方法、疗效和并发症。方法:分析24例婴儿肺切除手术患儿的临床资料。结果:24例中先天性肺囊肿10例、先天性大叶性肺气肿4例、先天性囊性腺瘤样畸形3例、肺隔离症1例,肺部感染合并脓胸6例。根据患儿情况行患侧肺叶、肺楔型或肺段切除术,其中5例同时行脓胸纤维板剥离术。24例患儿术程均顺利。术后并发支气管胸膜瘘、气胸、肺出血各1例,经治疗后好转;1例先天性肺囊肿于术后3日死于肺功能不全;其余20例术后恢复良好。生存的23例于术后8～15日出院。20例获得随访2个月～11年,除1例先天性大叶性肺气肿出现轻度气促外,其他病例无明显异常。结论:婴儿肺切除手术主要的病因为先天性疾病与严重肺部感染,应根据患儿实际情况选择肺切除手术的方式,并重视治疗术后并发症,处理得当手术疗效颇佳。Objective： To investigate the causes, methodology, efficacy and complications of lung resection in iiffancy, Methods： Clinical data of lung resection were reviewed in 24 cases. Results： Of 24 infants who underwent pulmonary resection, ten had congenital pulmonary cyst, four had congenital lobar emphysema, three had congenital cystic adenomatoid malformation, one had pulmonary sequestration, and six had pulmonary iiffection with empyema of the chest. Total lobectomy of lung, wedge type lobectomy or segment lobectomy of lung were performed according to the conditions of the iiffants, five cases accepted empyema fibrous lamina decollement at the same time. All operations were smooth, one died and twenty-three cured, the main complications were bronchopleural fistula, aeropleura~ empsyxis. The cured ones were discharged 8 - 15 days after operation. Twenty cases were followed- up for 2 months to 11 years, 19 cases were asymptomatic, 1 case with congenital lobar emphysema had mild respiratory distress. Conclusion： The diseases of infants underwent pulmonary resections include congenital disease and serious pulmonary infec- tion. Appropriate operation should be chosen according to the conditions of the iiffants, and the complications should be well treated. If well done, the curative effect of lung resection operation is good.
