乳腺黏液癌的钼靶影像特点及病理学基础

摘　　要：目的探讨乳腺黏液癌钼靶影像学表现的病理学基础。方法对66例乳腺黏液癌的钼靶影像与病理特点进行回顾性分析。结果所有影像均表现为肿物影或肿物伴钙化。单纯型35例,形态、边缘以良性特征为主,仅14%呈不规则形,29%边缘浸润,病理黏液含量高,误诊率为51%。混合型31例,形态、边缘以恶性特征为主,不规则形81%,边缘浸润、毛刺占88%,病理黏液量少,误诊率仅为9%。结论单纯型黏液癌多黏液组影像学表现类似良性疾病,少黏液组及混合型黏液癌多为浸润性生长的影像特点。Objective To evaluate the mammographic features and pathologic basis of mucinous carcinomas of the breast. Methods A retrospective analysis of the mammographic and pathologic findings of 66 patients with mucinous carcinomas of the breast was performed. Results The mammographic features of mucinous carcinoma were mass or mass with calcification. Shape and margin of pure mucinous carcinoma with high percentage of mucin were benign-like images. Only 14% of them were seen with regular shape, and 29% indistict margin. The misdiagnosis rate of pure type mucinous carcinoma of the breast was 51%. Mixed type mucinous carcinoma of the breast had malignant images, 81% with irregular shapes, 88% with defined margins or spiculate. Pathology exhibited a spot of extracellular mucin. The misdiagnosis rate of mixed type mucinous carcinomas of the breast was 9%. Conclusion The mammographic features of mucinous carcinoma of the breast correlate to different pathologic basis. The common mammographic appearances of pure mucinous carcinoma with high volume of mucin display as a benign lesion due to circumscribed margins. Pure type carcinomas with small volume of mucin and mixed type carcinomas have more aggressive imaging characteristics.
