摘  要:目的:建立能精确测定胰岛素敏感性的高胰岛素-正常葡萄糖钳夹技术.方法:应用高胰岛素-正常葡萄糖钳夹技术对10名正常体质量、正常糖耐量的健康志愿者进行方法学研究.结果:钳夹试验开始后,血浆胰岛素浓度迅速上升并维持在100mU/L左右,空腹血糖维持在正常水平(5.0mmol/L),试验过程中内源性胰岛素和肝糖源输出得到完全抑制,血液升糖激素浓度(包括皮质醇和生长激素)与基础值相比无明显上升.钳夹稳态期,葡萄糖输注率为(11.56±1.74)mg/(kg.min).结论:高胰岛素-正常葡萄糖钳夹技术已成功建立.在高胰岛素-正常葡萄糖稳态条件下,葡萄糖输注率等于机体的葡萄糖利用率,葡萄糖输注率可以作为评价组织对外源性胰岛素敏感性的指标.AIM : To establish a hyperinsulinemic-euglycemic clamp technique, a method for the precise measurement of insulin sensitivity. METHODS: A hyperinsulinemic-euglycemic clamp technique was applied to the study of methodology in 10 healthy volunteers with normal body weight and glucose tolerance. RESULTS: The serum insulin concentration was acutely raised and maintained at approximately 100 mU/L. During maintaining fasting euglycemia (5.0 mmol/L), the secretions of endogenous insulin and hepatic glucose production were completely inhibited. In comparison with basal values, the secretion of counter-regulatory hormones (including cortisol and growth hormone ) was not significantly stimulated. Under these steady-state conditions of hyperinsulinemia-euglycemia, the glucose infusion rate was ( 11.56 ± 1.74) mg/( kg · min). CONCLUSION: The hyperinsulinemiceuglycemic clamp technique is successfully established. Under these steady-state conditions of euglycemia, the glucose infusion rate equals to glucose disposal rate by all the tissues in the body and is therefore a index for tissue sensitivity to exogenous insulin.
