激光电脉冲穴位治疗糖尿病神经病变临床对比研究

摘　　要：目的:比较激光电脉冲穴位刺激治疗与常规改善循环、营养神经西药治疗糖尿病神经病变的疗效并探讨其治疗机理。方法:将95例糖尿病神经病变患者随机分成常规西药组(A组,n=32),激光电脉冲穴位刺激治疗组(B组,n=32)及两种方法联合治疗组(C组,n=31),观察三组的治疗效果、神经传导速度、血流变、血糖等指标。结果:(1)A组与B组相当,C组效果则明显加强,优于A、B组(P<0.05)。(2)A、B组治疗后神经传导速度较治疗前明显提高(P<0.05),两组间比较无显著差异(P>0.05),C组神经传导速度提高值较A、B组差异显著(P<0.05),而血流变值三组治疗前后均无差异(P>0.05),血糖值三组治疗前后差异显著,但组间比较均无显著差异(P>0.05)。结论:激光电脉冲穴位刺激治疗能有效改善糖尿病神经病变的临床症状,提高受损神经的传导速度,疗效等同于常规西药治疗,二者联合治疗后疗效明显加强。激光电脉冲穴位刺激治疗能克服针灸治疗时手工刺激强度不一、疼痛与皮肤破损不易愈合的缺点,为一种有效的治疗手段。Objective： To compare the therapeutic effect on diabetic neuropathy（DN） between photoimpact acupoint stimulation and western medicines（improving circulation and trophic nerve）, and to study the mechanism of photoimpact acupoint stimulation. Methods：Ninety-five cases of DN were randomly divided into three groups, 32 cases of photoimpact acupoint stimulation, 32 cases of western medicines, and 31cases of combined treatments. The therapeutic effects, nerve conduction velocity,blood rheologic index and blood sugar were observed in the three groups Results：（1）the therapeutic effect of combined group was best in the three groups（P〈0.05）（ 2） nerve conduction velocity was increased in the two single treatment groups（P〈0.05） ,but there was no statistical difference between two groups（P〈0. 05） among the three groups there was no difference in the blood rheologic indexs before and after treatment （P〉0.05） ; blood sugar level was significantly decreased after treatment in the three groups（P〈0.05） ,but there was no statistical difference among the three groups（P〉0.05）. Conclusion： Photoimpact acupoint stimulation could significantly improve the clinical symptoms of DN, increase conduction velocity of injured nerve,western medicine has the similar effects, however,their combination is the best. photoimpact acupoint stimulation is an effective therapeutic tool to DN and avoids the disadvantages of acupuncture therapy such as disparation intensity of handwork stimulation, pains and slowly wounds healing of skin.
