摘 要:目的探讨胸水腺苷脱氨酶(ADA)和血清抗结核抗体(anti-PPD-IgG)检测在结核性胸膜炎诊断上的价值。方法对2002~2006年确诊的结核性胸腔积液95例、恶性胸腔积液32例,漏出液16例患者的胸腔积液、血清进行ADA和anti-PPD-IgG检测,并对结果进行统计学分析。结果结核组与非结核组胸水ADA和血清anti-PPD-IgG检测均有显著差异(P<0.01),统计二项联合检测敏感性、特异性、准确性分别为91.6%、97.9%和93.7%,均高于单项检测(p<0.01)。结论联合检测胸水ADA和血清anti-PPD-IgG可提高结核性胸膜炎诊断的敏感性和诊断率。Objective To investigate the roles of chest fluid adenosine deaminase(ADA) assay and serum tuberculosis antibody (anti-PPD-IgG) detection in the diagnosis of tuberculous pleurisy. Methods All the patients were made a diagnosis in the department of respiratory of the Third Yangzhou People's Hospital between January, 2002 and December, 2006 which including three greups(95 cases of tuberculous pleural effusion, 32 cases malignant pleural effusion, 16 cases of transudate). All the patients accepted detection of two indexes mention above. Statistical analysis was made of the results. Results The levels of chest fluid ADA and the positive rate of serum anti-PPD-IgG in tuberculous pleurisy group were significantly higher than those in the non-TB group (P〈0.01). The sensitivity, specificity and accuracy of combination chest fluid ADA test with serum anti-PPD-IgG detection were 91.6%, 97.9% and 93.7%, respectively. Furthermore, these rate of combined method were obviously higher than those of the single test (P〈0.01). Conclusions Diagnostic sensitivity and diagnostic rate would be enhanced for patients with tuberculous pleurisy when combination the measurement of chest fluid ADA with serum anti-PPD-IgG detection.
