乙型肝炎患者血清细胞因子检测及其临床意义

摘　　要：目的探讨血清白细胞介素-6(IL-6)、IL-10和IL-12水平检测在乙型肝炎发病机制中的作用和临床意义。方法应用荧光定量PCR测定100例乙肝患者血清中HBVDNA含量,再分别采用酶联免疫吸附法(ELISA)和酶法检测患者血清IL-6、IL-10、IL-12和谷丙转氨酶(ALT)水平,并与100例正常健康者比较。结果乙型肝炎患者血清IL-6、IL-10、IL-12水平显著高于健康对照组(P<0.01),其中IL-6和IL-12水平与ALT呈明显正相关,而IL-10与ALT呈负相关。免疫治疗完全应答组血清HBVDNA含量与IL-6、IL-12水平呈正相关,而与IL-10水平呈负相关。免疫治疗完全应答组IL-6、IL-10、IL-12水平与免疫治疗无应答组有极显著性差异(P<0.01)。结论检测乙型肝炎患者血清IL-6、IL-10和IL-12水平可反应机体的免疫功能状态及肝功能的损害程度,并对预测患者的预后有一定的作用。Objective To study the clinical significance of serum interleukin-6 （IL-6）, IL-10 and IL-12 levels in patients with hepatitis B. Methods The levels of serum hepatitis B virus （HBV） DNA were detected by Fluorescence quantitation polymerase chain reaction, IL-6, IL-10 and IL-12 by enzyme linked immunosorbent assay （ELISA） and serum alanine aminotransferase （ALT） were also detected in 100 hepatitis B virus patients and 100 healthy donors. Results The levels of IL-6, IL-10 and IL-12 in HBV infected patients were significantly higher than those in healthy donors （P〈0.01）. Significant positive correlation was found between the levels of IL-6, IL-12 and the levels of ALT, respectively, but negative correlation was found between IL-10 and ALT. The positive correlation were also found between the levels of IL-6, IL-12 and the quantity of HBV DNA in the patients who had complete response to the treatments, but negative correlation was found between the levels of IL-10 and the quantity of HBV DNA. Simultaneously, the levels of IL-6, IL-10 and IL-12 in the responder patients were significandy higher than those in the non-responder patients （P〈0.01）. Conclusions Serum levels of IL-6, IL-10 and IL-12 in hepatitis B virus patients reflect the immune function and injury degree of livers and may play an important role in forecasting the therapeutic effects and prognosis of hepatitis B virus patients.
