碳酸化羟基磷灰石人工骨在去势大鼠疏松转子间注射后的降解和成骨反应

摘  要:目的:将自行研制的碳酸化羟基磷灰石人工骨注入骨质疏松大鼠转子间区,观察其降解和成骨情况。方法:实验于2005-05/2006-08在青岛市立医院实验中心完成。30只SD雌性大鼠去势,6个月后做成骨质疏松模型。将碳酸化羟基磷灰石人工骨注入一侧股骨转子间区,另一侧作为对照。于注射后1,3,7d和1,3,6个月分别处死5只,并截取股骨近段,制成切片,通过镜下观察及用组织形态学的方法观察碳酸化羟基磷灰石人工骨在骨质疏松骨质内的降解和成骨情况。同时用扫描电镜检测碳酸化羟基磷灰石人工骨与疏松骨质的整合情况。结果:30只全部进入结果分析。碳酸化羟基磷灰石注射侧出现成骨细胞和破骨细胞聚集现象,人工骨被吸收,大量新骨形成,注射后6个月矿化骨小梁面积较对照侧明显增加,证实碳酸化羟基磷灰石人工骨在大鼠疏松转子间区具有明显的骨传导现象。新生骨小梁与人工骨结合紧密,看不到由纤维组织填充的空腔,人工骨的吸收和新骨生成基本持平。结论:在疏松转子间区骨质内,碳酸化羟基磷灰石人工骨能够成功降解,同时具有明显的新骨生成反应。AIM: To determine the degradation and osteogenic response of the self-made carbonated hydroxyapatite (CHA) injected into the intertrochanteric region of rats with osteoporosis. METHODS: This experiment was done in the Experimental Center of Qingdao Municipal Hospital from May 2005 to August 2006. Thirty female SD rats were ovariectomized for osteoporosis, and 6 months later, osteoporosis rat models were experimentally induced. CHA cement was injected into unilateral intertrochanteric region, while the contralateral region was served as controls. Specimens of proximal femur were retrieved at 1, 3, 7 days and 1, 3, 6 months after injection, 5 rate being executed at every time point. The degradation and the osteogenic response of CHA cement were evaluated histomorphologically in osteoporotic bone. Meanwhile, the integration of CHA cement and osteoporotic bone was observed under scanning electron microscope. RESULTS: All of 30 rats were involved in the result analysis. There were more osteoblasts and osteoclasts appearing in the regions injected with CHA cement. CHA was absorbed and generous new bone came into being. The mineralized trabecular area in the groups injected with CHA was significantly bigger than that in the controls at 6 months, indicating that there were significant bone conductions of CHA cement in the intertrochanteric region of the osteoporotic rats. New trabecular bone contacted impact with CHA cement, and the resorption of CHA was fair to the new bone formation, without the presence of cavity filled with fibrous tissues. CONCLUSION: CHA cement can be degraded in the osteoporotic intertrochanteric region, and osteogenic response of CHA cement also occurs simultaneously.
