摘  要:目的:观察清热解毒复方(黄连清降合剂)抑制自发性高血压大鼠心肌纤维化的作用,并探讨其作用机制。方法:实验于2005-01/12在山东中医药大学细胞分子生物学实验室完成。①实验分组:将自发性高血压大鼠30只随机分为4组,即模型对照组6只、牛黄降压丸组8只、卡托普利组8只、黄连清降合剂组8只,并设Wistar大鼠8只作为正常对照组。②实验方法:牛黄降压丸组、卡托普利组、黄连清降合剂组分别给于牛黄降压丸混悬液、卡托普利混悬液、黄连清降合剂混悬液10mL/kg体质量,分别相当于0.75g牛黄降压丸/(kg·d)、35mg卡托普利/(kg·d)、22g中药生药/(kg·d)(均相当于临床用量的14倍量),灌服10mL/kg体质量;正常对照组和模型对照组给等量的生理盐水,不给任何药物。根据每周的体质量变化调整给药量。1次/d,每周用药6d,连续用药8周。③实验评估:采用套尾法测定各组大鼠清醒状态下尾动脉收缩压;采用苦味酸-天狼猩红染色法标记心肌胶原分布,并测定心肌胶原容积分数和血管周围胶原面积/管腔面积比;采用免疫组化法测定心肌Ⅰ、Ⅲ型胶原的表达;采用放射免疫分析法测定心肌血管紧张素Ⅱ的含量。结果:清热解毒复方(黄连清降合剂)能够降低自发性高血压大鼠清醒状态下尾动脉收缩压,而且无反弹和快速耐受性,能够减轻心肌间质和心肌内血管周围胶原纤维的沉积,能够降低心肌胶原容积分数和血管周围胶原面积/管腔面积比,能够降低心肌组织Ⅰ、Ⅲ型胶原的蛋白表达,能够降低心肌组织血管紧张素Ⅱ的含量,各指标效应与卡托普利组相近,均明显优于模型对照组(P<0.05)。结论:清热解毒复方(黄连清降合剂)具有良好的抗高血压心肌纤维化的作用,其机制与降低心肌组织血管紧张素Ⅱ的含量、抑制肾素-血管紧张素-醛固酮系统活性有关。AIM: To observe the effect of Huang/ian Oingjiang Mixture (HLQJM), compound of clearing heat and removing toxic substance, on cardiac muscular fibrosis of spontaneous hypertensive rat (SHR), and explore the action mechanism. METHODS: The experiment was performed in the Cell Molecular Biology Laboratory of Shandong University of Traditional Chinese Medicine from January to December 2005. ①Thirty SHRs were selected and randomly divided into 4 group: model control group (n =6), Niuhuang Jiangya Wan group (n =8), Captopril group (n =8), and HLQJM group (n =8). In addition, 8 Wistar rats served as normal control group. ②Niuhuang Jiangya Wan group, Captopril group, and HLQJM group were infused with 10 mL/kg separately suspension of corresponding medicine, 10 mL/kg body mass, equal to 0.75 g Niuhuang Jiangya Wanlkg daily, 35 mg Captopril/kg daily and 22 g crude drug/kg daily (equal to 14 times of clinical dosage), once, daily, 6 days a week for 8 weeks. The normal control group and model group were only given matching normal saline. The administration dosage was adjusted according to the changes in body mass. ③The tail arterial systolic pressure of conscious SHR was measured by tying tail method. The distribution of cardiac muscular collagen was marked by VG dye method and cardiac muscular collagen volume fraction (CVF) and ratio of perivascular collagen area and lumina area (PVCA) were detected. The expressions of cardiac muscular Ⅰ collagen and Ⅲ collagen were measured by immunohistochemical method, and the quantity of cardiac muscular angiotension Ⅱ by radioimmunoassay. RESULTS: HLQJM, the compound of clearing heat and removing toxic substance, reduced the tail arterial systolic pressure of conscious SHR with no rebound or tachyphylaxis, alleviate the accumulation of cardiac muscular collagen, diminish CVP and PVCA, down-regulate the expression of Ⅰ collagen and Ⅲ collagen, as well as reduce the quantity of angiotension Ⅱ in cardiac muscular tissue.
