高脂血症肾移植患者外周血血小板衍生生长因子BB及其受体mRNA表达及辛伐他汀治疗的影响

摘　　要：目的探讨肾移植术后高脂血症患者外周血小板衍生生长因子BB(PDGF-BB)及其受体PDGFβRmRNA的表达及辛伐他汀治疗的影响。方法根据患者肾移植术后6个月的血脂水平,从167例肾移植受者中随机选择60例作为研究对象,其中30例为血脂正常组(A组),30例为血脂增高组(B组),健康体检者30例作为对照组,逆转录聚合酶链反应检测各组外周血PDGF-BB及PDGFβRmRNA表达水平。血脂增高组给予辛伐他汀治疗,分别于1.5个月、3个月时复查血脂水平及PDGF-BB及PDGFβRmRNA表达水平。结果肾移植受者外周血PDGF-BB及PDGFβRmR-NA表达水平显著高于对照组,B组增高更为显著。辛伐他汀治疗1.5个月时,B组患者血脂水平及外周血PDGF-BB及PDGFβRmRNA表达水平显著降低,3个月时进一步降低。结论肾移植后高脂血症患者外周血PDGF-BB及PDGFβRmRNA表达水平显著增高,可能参与了移植后心血管疾病及慢性移植肾排斥的发生。辛伐他汀治疗可显著降低肾移植受者外周血PDGF-BB及PDGFβRmR-NA表达水平,此可能有助于减少或延缓移植后心血管疾病及慢性移植肾病的发生。Objective To investigate the mRNA expressions of PDGF-BB and PDGF βRin peripheral blood monocyte cells of renal transplant recipients and the influence by Simvastatin treatment. Methods Sixty recipients were selected from 167 renal transplant recipients, and were divided into two groups that Group A without hyperlipidemia; group B with hyperlipidemia. Control group consists of 30 healthy volunteers. The patients in group B were treated with Sirnvastatin for 3 months. The mRNA expressions of PDGF-BB and PDGF βR were detected with RT-PCR. Results The mRNA expressions of PDGF-BB and PDGF βR in post transplant recipients were significantly higher compared to normal controls, which it was more evident in group B than those in group A. Serum lipid levels in patients of groupB decreased significantly after Simvastatin treatment. Meanwhile, the mRNA expressions of PDGF-BB and PDGF βR reduced significantly at the 1. 5^th month of Simvastatin treatment. Conclusion It is suggested that up-regulated expressions of PDGF-BB and PDGF βR mRNA may be involved in the pathogenesis of CAN and CVD in renal transplant recipients with hyperlipidernia. Simvastatin might be used to reduce the mRNA expressions of PDGF-BB and PDGF βRin renal recipients with hyperlipidemia.
