摘  要:目的:探讨肾盂输尿管连接部梗阻段细胞增殖与神经分布状况。方法:应用免疫组织化学方法检测增殖细胞核抗原(PCNA)、神经肽Y(NPY)在28例肾盂输尿管连接部梗阻(病例组)和15例正常输尿管组织(对照组)的表达情况。结果:病例组PCNA阳性表达率85.71%(24/28),高于对照组的2/15(χ2=21.41,P<0.05);病例组PCNA阳性细胞主要位于黏膜层及纤维结缔组织层,4例阴性表达者存在肌层肥厚。病例组NPY阳性表达率为50%(14/28),低于对照组的86.7%(13/15),差异有统计学意义(χ2=5.62,P<0.05)。结论:肾盂输尿管连接部梗阻段细胞增殖和神经分布减少在先天性肾积水发病过程中起重要作用。Aim : To explore the cell proliferation and innervation situation in ureteropelvic junction obstruetion tissue. Methods:Proliferating cell nuclear antigen (PCNA) and neuropeptide Y (NPY) in ureteropelvic junction obstruction tissue from 28 eases of eongenital hydronephrosis(patient group) were detected using immunohistochemistry. A total of 15 cases of normal ureteropelvic tissue were the control. Results: The positive rate of PCNA in patient group was 86.71% (24/ 28 ) , higher than 2/15 in the control group(x^2 = 21.41, P 〈 0.05 ) ; The PCNA positive cells in patient group mainly located in mucous layer and fibrousconnective tissue, and the other 4 cases with PCNA negtive expression exited mucular. The positive rate of NPY in patient group was 50% (14/28) , lower than 86.7% ( 13/15 ) in the control group(x^2 = 5.62, P 〈 0. 05). Conclusion : Decrease of cell proliferation and innervation in ureteropelvic junction obstruetion tissue maybe play a significant role in congenital hydronephrosis pathophysiology.
