摘 要:目的调查1996-2005年杭州地区细菌性痢疾菌谱分布及其耐药情况,为临床合理用药提供依据。方法分析1 332株志贺菌属的菌型及对18种抗菌药物的耐药情况。结果检出弗氏志贺菌最高为1 235株,宋内志贺菌次之为97株;志贺菌属对氨苄西林、复方新诺明、氯霉素、诺氟沙星耐药严重,氨基糖苷类抗菌药物及喹诺酮类抗菌药物的耐药率较低,出现对第三代头孢类抗菌药物耐药及多重耐药现象,多重耐药的比例从1996年的6.0%上升到2005年的65.0%(P<0.01,χ2=6.14)。结论弗氏志贺菌是杭州地区优势菌,为减少耐药及多重耐药现象,临床治疗宜合理使用抗菌药物。OBJECTIVE To investigate the bacteral spectrum and drug resistance of bacillary dysentery in 1996 to 2005 in Hangzhou area to guide clinical treatment. METHODS The bacillary types of 1 332 strains of Shigella, and their drug resistance to 18 kinds of antibiotics were analyzed. RESULTS There were isolated 1 235 strains of S. flexneri and 97 strains of S. sonnei. The resistant rate to aminoglycosides and quinolones was much lower than to ampicillin and SMZ/TMP. Part of Shigella was resistant to third generation of cephalosporin antibitotics and with multiresistance. The proportion of multiresistance increased from 6.0% in 1996 to 65.0% in 2005 (P〈0.01, X^2=6.14). CONCLUSIONS The prevailing strains of Shigella are S. flexneri. Inorder to reduce the drug resistance rate and multireslstance, it is necessary to use drugs reasonably.
