慢性阻塞性肺部病变患者肺部CT灌注成像特征

摘　　要：目的探讨慢性阻塞性肺疾病(COPD)患者肺部CT灌注成像的时间密度曲线(TDC)和灌注参数特征。方法共44例,分为COPD组25例及对照组19例。均行肺部多层螺旋CT(MSCT)动态增强扫描。图像经后处理,设定感兴趣区(右腹肺、右背肺和左腹肺、左背肺),得到各处肺组织TDC和灌注参数,包括灌注值(P),强化峰值增量(PEI),达峰值时间(TTP)和血容量(BV)。比较分析COPD组和对照组的TDC特征,以及各灌注参数的差异性。结果COPD组较对照组TDC波峰延后。各灌注参数腹背肺比率在2组无显著性差异(P>0.05)。COPD组TTP较对照组明显延长(P=0.010)。P、PEI和BV在2组无显著性差异(P>0.05)。结论CT灌注成像显示COPD患者肺部血流灌注异常主要表现为TDC波峰延后,TTP延长。Objective To explore the characteristics of time - density curve （TDC） and perfusion indexes in pulmonary CT perfusion imaging（ MDCT - PPI） in patients with chronic obstructive pulmonary disease（ COPD）. Methods Totally forty - four cases were divided into two groups：25 cases in COPD group and 19 cases in control group were all undergone pulmonary CT dynamic contrast - enhanced scan. TDC and perfusion indexes including perfusion（P） ,peak enhancement increase（PEI） ,time to peak（TTP） and blood volume（BV） were obtained after putting the ROI in right ventral lung, fight dorsal lung,left ventral lung and left dorsal lung. TDC and the perfusion indexes were .statistically analysed between the two groups. Results Posterior wave crests of TDC were observed in the COPD group by comparing with the control group, There was no significant difference （P 〉 0.05 ） in the ventral to dorsal rates of indexes between the two groups, TTP in COPD group was longer significantly （P = 0. 010） than that in control group. There was no significant difference in P, PEI and BV between the two groups. Conclusion The posterior wave crests of TDC and longer TTP are the main features on pulmonary CT perfusion imaging in the patients with COPD.
