摘 要:目的:探讨肾综合征出血热(HFRS)患者的早期实验室诊断。方法:分别利用酶联免疫吸附(ELISA)法检测HFRS患者抗体IgM,免疫荧光法(IFA)检测患者抗体IgG,套式RT-PCR法检测患者血清中的病毒RNA,同时对PCR产物M基因部分片段进行序列分析。结果:16例发病1周以内的患者的血清样本中,IgM阳性比例为9/16,IgG阳性比例为9/16,2者均阳性者为7例,IgM或IgG最早出现在发病后第4天。16例患者中RT-PCR阳性表达有11例,其中在发病≤5d的患者样本中,RT-PCR阳性比例达到11/12,基因分型均为SEO型。结论:相对于血清学检测,利用RT-PCR进行HFRS患者的病毒核酸检测更有利于早期诊断及分子流行病学调查。Objective: To study the early laboratory diagnosis of hemorrhagic fever with renal syndrome (HFRS). Methods: The specific IgM and IgG of the patients were detected by ELISA and IFA respectively. The hantavirus RNA in the patients was amplified by nested RT-PCR and the PCR products of the viral M segment were sequenced. Results: Positive rates of IgM and IgG were 9/16 and 9/16 respectively and 7 patients were postive in IgM and IgG detection among 16 patients. The IgM or IgG antibody of patients attacked with HFRS was detected on the fourth day. The virus RNA were detected and belonged to SEO type from 11 of 16 patients and RT-PCR positive rate was 11/12 in 12 patients at early stage(≤5 days). Conclusion: Compared with serological test,hantavirus RNA detection by RT-PCR is more useful in HFRS early diagnosis and molecular epidemiology.
