贫血与重度慢性心力衰竭预后的相关性研究

摘　　要：目的:探讨贫血与重度慢性心力衰竭(CHF)预后的相关性。方法:将心功能为Ⅲ～Ⅳ级(NYHA分级)的慢性心力衰竭患者按血红蛋白(Hb)水平将其分为Hb正常组、轻度贫血组及中度贫血组,采用前瞻性队列研究的方法,在接受常规治疗心力衰竭情况下,追踪观察3年,收集有关资料,分析贫血对重度CHF患者再住院率和病死率等指标的影响。结果:合并贫血的重度CHF患者的再住院率、病死率高于不合并贫血的患者(P<0.05);中度贫血的重度CHF患者的再住院率、病死率高于轻度贫血组及Hb正常组(P<0.05);3年后中度贫血的重度CHF患者其左室射血分数(LVEF)、左室舒张末期前后径(LVEDV)及6min步行距离与住院时及正常组比较其心功能更差(P<0.05)。结论:重度CHF合并贫血的患者其再住院率及病死率较高;且贫血的程度越重,其病情也越严重,预后越差。Objective： To study the correlativity of anaemia and the prognosis of serious chronic heart failure. Methods： The CHF patients with Ⅲ-Ⅳ level（NYHA）received in 2002 were divided into three groups including the Hb normal group,the mild anaemia group and the moderate anaemia group according to the Hb level. Under the convention control heart failure treatment,related data were collected and the effect of anaemia on again being hospitalized rate and mortality were analyzed in the three year follow up. Results： The again being hospitalized rate and mortality in CHF patients combining anaemia were higher than those in CHF patients not combining anaemia（P 〈 0.05）. Again being hospital rate and mortality in moderate anaemia serious CHF patients were higher than those in mild anaemia group and the Hb normal group（P 〈 0.05）. The left ventricles ejection fraction（LVEF）,left ventricular end-diastolic volume（LVEDV）and 6 mins walking distance in moderate anaemia serious CHF patient were compared with those in the normal group and when they were hospitalized and their heart function were worse（P 〈 0.05）. Conclusion： The agaed being hospitalized rate and mortality of serious CHF patients are higher,the anaemias degree is more deeply,the state of illness is more serious,and the prognosis is worse.
