多平面经食管三维超声心动图与左室造影测量左室收缩功能的对比研究  

A Comparative Study of Multiplane Transesophageal Three-dimensional Echocardiography and Left Ventriculography in Measuring Left Ventricular Systolic Function

摘　　要：18只实验犬于冠状动脉前降支结扎前后，进行了多平面经食管三维超声心动图（3DE）、心外膜二维超声心动图（2DE）和左室造影的同期对比研究，应用三种技术测量左室舒张末期容量（EDV）、收缩末期容量（ESV）、心搏量（SV）和射血分数（EF）。结果显示：冠脉结扎前，2DE和左室造影所测EDV、ESV、SV和EF的相关系数分别为0.90、0.93、0.88和0.94，3DE和左室造影的相关系数分别为0.94，0.94，0.92，和0.96;冠脉结扎后，2DE和左室造影测值的相关程度显著下降（r分别为0.84，0.82，0.80和0.85），但3DE和左室造影的相应测值仍高度相关（r分别为0.89，0.87，0.87和0.91），表明当左室形态正常时，2DE和3DE均可准确测量左室容量和收缩功能，但当左室出现显著节段性运动异常时，3DE的准确性明显高于2DE，从而为冠心病左室收缩功能的测量提供了简便可靠的新途径。To evaluate the accuracy of newly-developed multiplane tansesophageal three-dimensional echocardiography (3DE) in measuring left ventricular volume and systolic function, epicardial two-dimensional echocardiography (2DE), 3DE and left heart catheterization (CATH) were performed in 18 dogs before and after coronary artery ligation.The three-dimensional left ventricle was reconstructed from 19 cross sectional images which were obtained by rotating the phased array of the transesophageal probe every 10° from 0°-180°. Left ventricular end-diastolic volume (EDV), end systolic volume(ESV), stroke volume (SV) and ejection fraction(EF) were measured from 2DE tising apical biplane Simpson's formula, from 3DE using a 3-D reconstruction software program and from left ventriculography using Simpson's formula. The result showed that EDV, ESV, SV and EF measured by 2DE and CATH correlated highly before coronary artery ligation (r=0. 90, 0. 93, 0. 88 and 0. 94, respectively), but only moderately after coronary ligation. (r=0. 84, 0. 82. 0. 80 and 0. 85, respectively). The correlations of EDV, ESV, SV and EF measured by 3DE and CATH were also high before coronary ligation (r=0. 94, 0. 94, 0. 92 and 0. 96, respectively) and remained high even after coronary ligation (r=0. 89,0. 87, 0. 87 and 0. 91, respectively). In conclusion, both 2DE and 3DE are reliable techniques for measuring left ventricular volume and systolic funetion in normal-shaped left ventricles without regional wall motion abnormalities (RWMA).However, 3DE is more accurate than 2DE in measuring these indices in myocardial ischemia with RWMA.
