经胸动态三维超声心动图在临床应用方面的探索性研究

摘　　要：本研究探索了经胸动态三维超声心动图重建的方法学、及其在临床诊断方面的可行性、可靠性及应用价值。仪器主要采用了Accuson128XP／10C彩色多功能超声心动图仪及变频探头及TomTec公司超声扫描三维成像系统。共选择正常人及各种心脏病患者49例，其中正常人6例，风湿性心脏病8例，各种先天性心脏病33例，其他患者2例。94幅图像中，3幅因探头移位或图像采集条件设置欠佳外，均获得较为满意的三维图像，如正常人的主动脉瓣及二尖瓣口，风湿性心脏病狭窄的二尖瓣口、房、室间隔缺损处的具体形态，I型共同动脉干主肺动脉从主动脉发出的部位及管腔的内经等，并重建了分流性疾病的血流状况。图像立体感强，直观与实体解剖十分相近。且经胸三维重建方法与经食管三维重建方法相比简便易行，无创，患者痛苦小，并可提供心脏各部位的立体解剖图像，有很大的临床应用价值。To study the method of transthoracic real-time three-dimensional echocardiography and to evaluate its possibility, feasibility and value in clinical application, color multi-function echocardiographic imaging system (Accuson 128 XP/10C) interfaced to a dedicated 3-D image processing computer unit (Tomtec inc. ) was performed for 6 cases with normal hearts and 43 with various heart disorders, namely & rheumatic heart disease, 33 congenital heart disease and others.Forty-nine cases were reconstructed satisfactorily by 3-D echocardiographic images, which showed narrowed mitral valve orifice, detective atrial or ventricular septum and reconstructed blood shunting. These images displaying cardiac tomographic views were very similar to that of cardiac anatomy. As Compared with transthoracic and transesophageal 3-dimensional echocardiography, transthoracic method was a simple, noninvasive, with high quality 3-D pictures of cardiac anatomy greatly valuable for clinical application.
