蝮蛇抗栓酶对糖尿病氮质血症期的疗效观察

摘　　要：研究蝮蛇抗栓酶（ＳＶＡＴＥ）对糖尿病肾病（ＤＮ）氮质血症期肾功能的影响。方法：将血液流变学异常的ＤＮ氮质血症期患者４２例随机分为２组：常规治疗组，包括糖尿病饮食控制及优质低蛋白饮食；另一组除常规治疗外加用ＳＶＡＥＴ静点，３疗程后比较两组肾功能改变。结果：尿白蛋白、肾小球滤过分数与高切还原粘度、低切还原粘度、纤维蛋白原、胆固醇、甘油三酯呈明显正相关。肾血浆流量上升、肌酐清除率无变化。ＳＶＡＴＥ组的这种相关性较常规治疗组更为明显（Ｐ＜０．００１）。结论：常规治疗基础上加用ＳＶＡＴＥ对ＤＮ氮质血症期患者能改善肾内血流动力学紊乱、减轻并延缓ＤＮ肾衰进展。Objective:The purpose of this study was to explore the effects of snake venum antithrombus enzyme(SVATE) on the renal function in patients with diabetic nephropathy azotemia.Method:Fouty-two cases of diabetic nephropathy azotemia with abnormal hemorheologic properties enrolled in the study,were randomly divided into two groups:normal treatment group receiving a diabetic diet and low protein diet,and SVATE group receiving routine treatment plus SVATE.The renal functions of these two groups were compared.Results:The urinary albumin,glomerular filtration fraction were positively related to the viscosity at high shear rate,the viscosity at low shear rate,fibin level,and the levels of cholesterol and friglyceride.No significant changes were found in Ccr but effective renal plasma flow rose.The correlations in the SVATE group were more remarkeble than those in the normal treatment group( P <0.01).Conclusion:Normal treatment plus SVATE is helpful in delation of diabetic nephropathy azotemia.
