应用二甲双胍治疗多囊卵巢综合征的临床观察

摘　　要：【目的】探讨二甲双胍(metformin)在治疗多囊卵巢综合征(Polycystic ovary Syndrome,PCOS)中的作用及机制。【方法】84例PCOS患者给予二甲双胍500 mg,3次/天,连续3个月,采取自身对照,观察用药前、后临床症状,血清卵泡刺激素(FSH)、黄体生成激素(LH)、雌二醇(E2)、睾酮(T)、雄烯二酮(A2),空腹血糖及空腹胰岛素水平的变化。【结果】月经恢复率为60.7%(51/84),排卵率42.9%(36/84),妊娠率25%(21/84),用药后空腹胰岛素和雄激素水平均降低,有显著性差异(P<0.05)。【结论】二甲双胍可以改善P-COS患者的胰岛素抵抗状态,降低血中雄激素水平,并能一定程度恢复生育功能。[Objective]To investigate the effect and mechanism of metformin in patients with polycystic ovary syndrome（PCOS）.[Methods]Eighty four patients with PCOS were selected to take metform at a dose of 500mg three times a day for 3 months,and the changes of their clinical features,reproductive hormones（FSH,LH,E2,T,A2） and glucose metabolisms level（fasting blood glucose,fasting blood insulin） were observed.[Results]Fifty one out of 84 had more regular menstrual cycles,36 patients had ovulation and 21 patients conceived.The fasting blood insulin and T level were decreased significantly after treating.[Conclusion]Metformin may effectively restore conceptive capability of PCOS,which may be related to its improvement on insulin resistance and decreasing T level.
