摘  要:目的探讨盐酸氨溴索气管内滴入在新生儿机械通气中的临床应用疗效。方法将40例患儿随机分成2组,对照组采用气管内滴入生理盐水,观察组采取气管内滴入稀释的盐酸氨溴索。结果治疗前2组动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)比较无显著差异(P>0.05),治疗后观察组PaO2明显高于对照组(P<0.01),PaCO2明显低于对照组(P<0.01);观察组呼吸机相关性肺炎发生率(10%)明显低于对照组(25%)(P<0.05);观察组机械通气时间(3.05±1.33)d明显短于对照组(5.21±1.38)d(P<0.05);2组均无明显不良反应发生。结论盐酸氨溴索气管内滴入在新生儿机械通气中可减少气管黏液滞留,促进排痰,改善通气,防治肺部感染,缩短机械通气时间。Objective To observe the effects of clinical application of dripping ambroxol hydrochloride into airway in newborns with mechnical ventilation. Methods Forty newborns with mechanical ventilation were divided into two groups randomly. Dripping sodium chloride into airway in control group and dripping ambroxol hydrochloride into airway in observasion group were carried out. Results There was no significant deviation in partial pressure of oxygen in artery ( PaO2 ) and partial pressure of carbon dioxide in artery( PaCO2 ) between two groups before treatment, after treament, PaO2 was higher in observation group than this in control group( P 〈 0.01 ) ,PaCO2 was lower in observation group than this in control group( P 〈0.01 ). The incidence of ventilator-associated pneumonia was lower in observation group( 10% ) than this in control group( 25%, P 〈 0.05 ), the time of mechnical ventilation was shorter in observation group(3.05 ± 1.33 d) than this in control group(5.21 ± 1.38 d,P 〈 0.05). There was no obvious adverse effect in two groups. Conclusion Ambroxol can obviously improve sputa viscosity, promote excretion of secreta from respiratory tract, prevent pneumonia infection and shorten the time of mechanical ventilation.
