骨折延迟愈合患者外周血骨形态发生蛋白-2水平的检测

摘　　要：目的：探讨骨折延迟愈合患者外周血骨形态发生蛋白-2（BMP-2）水平的变化与其临床特征之间的关系。方法：73例骨折患者应用ELISA方法对其骨折后l、2、4、8、12、24、36、48、60周进行外周血的BMP-2检测,检测结果按照骨折延期愈合与正常愈合,延期愈合组中又按照骨折部位、对常规治疗的反应效果进一步分组,统计这些组别外周血中BMP-2的不同表达水平。结果：骨延迟愈合患者外周血BMP-2在创伤后第一周内迅速升高,第2周内迅速回落,而后维持在比正常人群还低的水平;骨正常愈合患者中第1~2周缓慢升高,至第8周才开始回落。将骨延迟愈合患者细分化后发现四肢骨近端部位外周血BMP-2表达规律接近于总骨延迟愈合患者外周血BMP-2随时间表达规律。而四肢骨远端包括掌指骨外周血BMP-2在骨折后一直未见增高。常规治疗48周后临床愈合的患者其外周血BMP-2时间表达规律与正常对照组相似,而有效和无效组外周血BMP-2时间表达规律则与总骨延迟愈合患者外周血BMP-2时间表达规律。结论：骨折后外周血中BMP-2表达水平与骨折愈合情况及骨折部位有关。Objective To investigate the relationship between serum bone morphogenetic protein-2（BMP-2） level and clinical characteristics in patients with delayed fracture healing.Method The serum BMP-2 level were detected at l,2,4,8,12,24,36,48,60 weeks with ELISA assay after frature and such patients were divided into delayed fracture healing group and normal fracture healing group,and the former group even divided according to fracture position and the effect to conventional therapy.The serum BMP-2 level were observed.Results The serum BMP-2 level increased in the first week,decreased in the second week and maintained low level after fracture in delayed fracture healing group;the serum BMP-2 level increased slowly in the first two weeks and decreased in eight weeks after fracture in normal fracture healing group.The serum BMP-2 expression rule in proximal end of limbs bone and bad effect to conventional therapy showed similar to con-delayed fracture healing,the serum BMP-2 maintained low level after fracture in distal end of limbs bone and excellent effect to conventional therapy showed similar rule with normal fracture healing.Conclusion The serum BMP-2 level shows correlation with situation of fracture union and fracture position.
