降压胶囊联合尼莫地平对老年单纯收缩期高血压患者血管内皮细胞功能的影响

摘　　要：目的:观察降压胶囊联合尼莫地平对老年单纯收缩期高血压(EISH)患者的降压疗效及其对血管内皮细胞功能的影响。方法:采用随机、双盲、对照的试验设计方法,将120例EISH患者随机分为治疗组与对照组,治疗组口服降压胶囊与尼莫地平胶囊,对照组口服尼莫地平胶囊与降压胶囊模拟药,疗程为4周。观察两组患者治疗前后的血压值与临床症状变化,测定血清一氧化氮(NO),血浆内皮素-1(ET-1)、血栓素B2(TXB2)与6-酮-前列腺素F-1α(6-Keto-PGF-1α)治疗前后的含量变化。结果:治疗组对收缩压的疗效明显优于对照组,组间比较有显著性差异(P<0.05);两组对舒张压的影响无显著性差异。治疗组降压显效15例,有效29例,无效10例,总有效率81.5%,明显优于对照组(P<0.01)。治疗组中医症状疗效总有效率为66.7%,也显著优于对照组(P<0.01)。治疗组血清NO及血浆6-Keto-PGF-1α水平较对照组均明显升高(P<0.05),而血浆ET-1、TXB2水平较对照组显著降低(P<0.05)。结论:降压胶囊联合尼莫地平对EISH患者有较好的降压与症状改善作用,可显著改善患者血管内皮细胞功能。Objective： to observe the effect of Jiangya capsule combined with nimodipine on depressurization and endothelial function of elderly isolated systolic hypertension（EISH） patients. Methods： randomized, blind and control test was applied. 120 EISH patients were divided randomly into treatment group and control group. Treatment group was treated with Jiangya capsule and nimodipine, control group was treated with nimodipine and Jiangya capsule placebo. The duration of treatment was 4 weeks. To observe the blood pressure and clinical symptoms, and detect serum NO, plasma TXB2,6-Keto-PGF-1α and ET-1 before and after the test. Results ：SBP was decreased significantly in treatment group compared with control group（ P 〈 0.05）, and there was no difference on DBP of the two groups. In treatment group, 15 cases were significantly effective, 29 cases were effective, 10 cases were ineffective, total effective rate was 81.5% and was superior to control group （P〈0.01）. Total effective rate of TCM symptoms in treatment group was 66.7%, and was superior to control group（P〈0.01）. Serum NO, plasma 6-Keto-PGF-1α were elevated and plasma ET-1, TXB2 were decreased in treatment group compared with control group（ P 〈 0.05）. Conclusion： Jiangya capsule combined with nimodipine have good effect on depressuriation clinical symptoms, and can improve vascular endothelial function of the patients.
