摘 要:目的取得螺旋CT图像上东北地区正常成人肺叶肺段支气管壁厚度,为引起支气管壁增厚疾病的诊断,特别是中心型肺癌的较早期诊断提供数据参考。方法对104例正常成人肺部螺旋CT扫描的中肺野区横断面图像行1mm重建层厚、1mm重建间隔和多平面重建,测量肺段及肺段以上支气管壁的厚度,并对27例中心型肺癌和11例支气管内膜结核的CT图像进行相同的后处理和测量,结果肺段及肺段以上支气管壁正常厚度从3.87±0.35mm到2.45±0.5mm,各年龄组间及性别间无显著性差异。27例中心型肺癌和11例支气管内膜结核的病变支气管厚度与相应的正常值统计学有显著性差异(P<0.02),27例中心型肺癌与11例支气管内膜结核的病变支气管厚度统计学有非常显著性差异(P<0.001)。结论螺旋CT图像上东北地区正常成人肺叶肺段支气管壁厚度正常值对引起支气管壁增厚疾病的诊断有重要的参考价值。Objective To obtain the normal thickness of the lobar and segmental bronchial wall in adults of the Northeastern region of China, and provide the data for the diagnosis of some diseases which cause the thickening of the bronchial wall, especially in the earlier diagnosis of the central lung cancer. Methods 104 case normal adults axial images in the middle part of the lung field had been reconstructed at 1 mm thickness and 1 mm intervals and a multi-planar reconstructions were performed on GE Prospeed AI scanner after 10 mm collimation, pitch of 1.The thickness of the lobar and segmental bronchial wall demonstrated had been viewed and measured on the spiral CT images by two radiologists separately or commonly. 38 patients including central lung cancer (n=27), endobronchial tuberculosis (n=11), all the final diagnosis, pathologicaly proved, were performed in the same way. Results The normal thickness of the lobar and segmental bronchial wall was from 3.87 ± 0.35 mm to 2.45 ± 0.5 mm . There were no distinct difference in diverse age groups and sexes. In central lung cancer and endobronchial tuberculosis groups the thickness was from 10.67±2.7 mm to 4.5 mm and from 11.00 mm to 4.00 mm , which was markedly different from the normal thickness (P〈0.001,P〈0.02). There was a significant difference (P〈0.001) between central lung cancer and endobronchial tuberculosis. Conclusion The normal thickness of the lobar and segmental bronchial wall in adults of the Northeastern region of China was more helpful the diagnosis of the diseases causing the bronchial wall thickening.
